Group Life and Extended Health Benefits Plan. (a) The Employer shall pay fifty (50) percent of the premium of an extended Health and Group Life Insurance Plan and the employee shall pay fifty (50) percent. (b) When an employee is on extended leave without pay, then the employee may pay the full premium in order to maintain coverage while on such leave. (c) Employees on maternity/adoption leave will have the option of continuing to pay their portion of the group insurance plan premiums to a maximum of fifty-two (52) weeks. Where the employee opts to continue to pay these premiums, the Employer will pay its share of the premiums. (d) A summary of the general provisions and benefits of the plan is appended to this agreement.
