Health Insurance/Life Insurance. During the Term, Resident will be entitled to participate in standard health insurance plan(s) and life insurance plan(s) for Resident and Resident’s eligible dependents as afforded by Broward Health, as set forth in and governed by the GME Policies and Procedures and Broward Health Human Resources Benefits Manual.
Health Insurance/Life Insurance. Employer shall provide Executive and his wife medical insurance coverage under Employer's group medical plan during the term of this Agreement and (if longer) until Executive and his wife have each become eligible for Medicare coverage, and Employer shall provide medical insurance coverage supplemental to Medicare coverage consistent with the coverage of Employer's group medical plan until Executive reaches the age of 65 years, subject to Executive's making of any reasonably required premiums or contributions consistent with the requirements of Employer's group medical plan. Employer shall also provide Executive a life insurance policy with a death benefit of at least $500,000 to extend until Executive reaches the age of 65 years, the beneficiary or beneficiaries of such policy to be as specified by Executive from time to time. The provisions of this Section 2.5 shall survive the Employment Term and any termination of Executive by Employer except a termination for "just cause" as defined in Section 3.3.

Additional insurance benefits which are made available to County employees at large shall also be made available to members of the bargaining unit. Employees should consult applicable sections of Webb county Civil Service Policies Manual, as amended, or contact the Webb County Administrative Services Department for further information in this regard.
Health Insurance/Life Insurance. Full-time bus drivers may, at the expense of the individual driver, and if eligible to participate in the Employer’s group health insurance program, institute a payroll deduction authorization for the deduction of such premiums during the school year. The driver who desires to continue coverage through the summer months shall make arrangements with the payroll clerk for deductions during that period or shall pay such premiums to the Employer according to the premium payment schedule for the school district. The employer will pay $100 per month toward participation in the Employer’s group health insurance program for all drivers who choose to participate in the Employer selected health insurance program. All bus drivers shall be provided with $18,000 Term Life Insurance with the carrier to be selected by the Board. The Mechanic and Mechanics Helper will receive medical insurance benefits that are comparable to or exceed the benefits provided to the administrative staff. This insurance shall include health, vision, and dental. (93% of the total cost of the premium will be paid by the District, 7% co-payment will be the responsibility of the employee)

  • Key Man Life Insurance The Company may apply for and obtain and maintain a key man life insurance policy in the name of Executive together with other executives of the Company in an amount deemed sufficient by the Board, the beneficiary of which shall be the Company. Executive shall submit to physical examinations and answer reasonable questions in connection with the application and, if obtained, the maintenance of, as may be required, such insurance policy.

  • Health Insurance Benefits To the extent provided by the federal COBRA law or, if applicable, state insurance laws, and by the Company’s current group health insurance policies, Executive will be eligible to continue Executive’s group health insurance benefits at Executive’s own expense. Notwithstanding the foregoing, if the Company determines, in its sole discretion, that it cannot provide the foregoing benefit without potentially incurring financial costs or penalties under applicable law (including, without limitation, Section 2716 of the Public Health Service Act), the Company shall in lieu thereof provide to Executive a taxable monthly amount to continue his group health insurance coverage in effect on the date of separation from service (which amount shall be based on the premium for the first month of COBRA coverage), which payments shall be made regardless of whether Executive elects COBRA continuation coverage and shall commence in the month following the month in which Executive incurs a separation from service and shall end on the earlier of (x) the date on which Executive voluntarily enrolls in a health insurance plan offered by another employer or entity during the period in which the Company is paying such amounts and (y) 18 months after the date of Executive’s separation from service.
