Common use of Hospital-Surgical Clause in Contracts

Hospital-Surgical. For the duration of this Agreement, the County shall provide a comprehensive hospital-surgical-major medical coverage policy and a $25 deductible dental plan. The County will continue to provide a Dental Maintenance Organization (currently Dental Associates) with a $2,200 annual cap, a $20 co-pay and a 50% split on orthodontia. Effective January 1, 2007, the Dental Associates annual cap will increase to $2,500 per eligible participant. Active employees will have the option of choosing one of two Network options, In-Network or Out-of-Network. Said option must be executed during the open enrollment period which will last for one month, from October 1 through October 31 of the current year. In Network Out-of-Network
