Information Regarding Key Personnel in Proposal. The individuals with direct responsibility for each of the following categories of work are considered Key Personnel: • overall management of the Project (Project Manager); • design of the Project (Design Manager); • control of quality and the implementation and operation of the Design Quality Management Plan (DQMP) (Professional Services Quality Control Manager (PSQCM) as described in Section 2.2.7.4.1 of the Technical Provisions); • independent design quality assurance and review of quality control measures in accordance with the DQMP (Design Quality Assurance Manager (DQAM) as described in Section 2.2.7.4.2 of the Technical Provisions); • construction, coordination of subcontractors and scheduling (Construction Manager); • control of quality, and the implementation and operation of the Construction Quality Management Plan (CQMP) (Construction Quality Control Manager (CQCM) as described in Sections 2.2.6 and 2.2.8.1.1 of the Technical Provisions); • independent quality acceptance for construction and review of quality control measures in accordance with the CQMP (Construction Quality Acceptance Manager (CQAM) as described in Section 2.2.8.1.3 of the Technical Provisions); • environmental compliance (Environmental Compliance Manager); and • Project maintenance management and maintenance quality control (Capital Maintenance Manager as described in the CMA).

  • PERSONNEL DISCLOSURE 2 26.1 CONTRACTOR shall make available to ADMINISTRATOR a current list of 3 all personnel providing services hereunder, including résumés and job 4 applications. Changes to the list will be immediately provided to 5 ADMINISTRATOR in writing, along with a copy of a résumé and/or job 6 application. The list shall include:

  • CERTIFICATION REGARDING CERTAIN FOREIGN-OWNED COMPANIES IN CONNECTION WITH CRITICAL INFRASTRUCTURE (Texas law as of September 1, 2021) By submitting a proposal to this Solicitation, you certify that you agree to the following required by Texas law as of September 1, 2021: Proposing Company is prohibited from entering into a contract or other agreement relating to critical infrastructure that would grant to the company direct or remote access to or control of critical infrastructure in this state, excluding access specifically allowed by the Proposing Company for product warranty and support purposes. Company, certifies that neither it nor its parent company nor any affiliate of company or its parent company, is (1) owned by or the majority of stock or other ownership interest of the company is held or controlled by individuals who are citizens of China, Iran, North Korea, Russia, or a designated country; (2) a company or other entity, including governmental entity, that is owned or controlled by citizens of or is directly controlled by the government of China, Iran, North Korea, Russia, or a designated country; or (3) headquartered in China, Iran, North Korea, Russia, or a designated country. For purposes of this contract, “critical infrastructure” means “a communication infrastructure system, cybersecurity system, electric grid, hazardous waste treatment system, or water treatment facility.” See Tex. Gov’t Code § 2274.0101(2) of SB 1226 (87th leg.). The company verifies and certifies that company will not grant direct or remote access to or control of critical infrastructure, except for product warranty and support purposes, to prohibited individuals, companies, or entities, including governmental entities, owned, controlled, or headquartered in China, Iran, North Korea, Russia, or a designated country, as determined by the Governor.

  • Correction of Personal Information 7. Within five (5) business days of receiving a written direction from the Local Government to correct or annotate any personal information, the Supplier must annotate or correct the information in accordance with the direction.

  • Sharing Information with Billers You authorize us to share identifying personal information about you (such as name, address, telephone number, Xxxxxx account number) with companies that you have identified as your Billers and which we have identified as offering electronic bills for purposes of matching your identity on the Service’s records and the Xxxxxx’x records to (a) activate your affirmative request for electronic bills, and/or (b) confirm your eligibility for “trial basis” electronic bills.

  • Additional Information for Product Development Projects Outcome of product development efforts, such copyrights and license agreements. • Units sold or projected to be sold in California and outside of California. • Total annual sales or projected annual sales (in dollars) of products developed under the Agreement. • Investment dollars/follow-on private funding as a result of Energy Commission funding. • Patent numbers and applications, along with dates and brief descriptions.  Additional Information for Product Demonstrations: • Outcome of demonstrations and status of technology. • Number of similar installations. • Jobs created/retained as a result of the Agreement.
