Insured Benefits. (The following clause is applicable to full-time employees only) (a) The Hospital agrees to pay 100% of the billed premium towards coverage of eligible employees in the active employ of the Hospital under the Blue Cross Semi-Private Plan in effect as of September 28, 1993 or comparable coverage with another carrier. (b) The Hospital agrees to contribute 75% of the billed premium towards coverage of eligible employees in the active employ of the Hospital under the existing Blue Cross Extended Health Care Benefits Plan in effect as of September 28, 1993 (as amended below) or comparable coverage with another carrier providing for $22.50 (single) and $35.00 (family) deductible, providing the balance of monthly premiums is paid by the employee through payroll deductions. Vision care maximum $200.00 and an eye exam every 24 months and hearing aide allowance acquisition every 36 months. Reimbursement for prescribed drugs covered by the Plan will be based on the cost of the lowest priced therapeutically equivalent generic version of the drug, unless there is a documented adverse reaction to the generic drug. (e) The Hospital will provide equivalent coverage to all employees who retire early and have not yet reached age 65 and who are in receipt of the Hospital's pension plan benefits on the same basis as is provided to active employees for semi-private, extended health care and dental benefits. Services of a chiropractor will be covered up to an annual maximum of $375 and services of a licensed or registered physiotherapist will be covered up to an annual maximum of $375. Vision care (maximum of $300 every 24 months plus bi-annual eye exams, which may be applied to laser eye surgery) as well as a hearing aid allowance (cost of acquisition per individual every 36 months.) Existing provisions for private duty nursing services contained in present extended health care plans will be amended to reflect that this benefit is limited to a maximum of ninety (90) eight-hour shifts in any calendar year.
(c) The Hospital Company agrees to contribute pay one-hundred percent (100% %) of the billed premium towards coverage of eligible employees in the active employ of the Hospital Company under HOOGLIP in effect as of September 28, 1993 Manulife or such other group life insurance plan currently in effect providing the balance of the monthly premium is paid by the employee through payroll deductionsdeduction.

(e) The Hospital will provide equivalent coverage to all employees who retire early and have not yet reached age 65 and who are in receipt of the Hospital's pension plan benefits on the same basis as is provided to active employees for semi-private, extended health care and dental benefits. (b) The Hospital agrees to contribute seventy-five percent (75% %) of the billed premium premiums towards coverage of eligible regular full time employees in the active employ of the Hospital under the existing Blue Cross Extended Health Care Benefits Plan in effect as of September 28, 1993 (as amended below) Sun Life benefits or comparable coverage with another carrier providing for $22.50 15.00 (single) and $35.00 25.00 (family) deductible, providing the balance of monthly premiums is are paid by the regular full time employee through payroll deductions. Vision In addition to the standard benefits, coverage will include vision care (maximum $200.00 and an eye exam 250.00 every 24 months, effective June 1, 2006) which will include eye exams and/or may be applied to offset cost of laser eye surgery as well as a hearing aid allowance (lifetime maximum $500.00 per individual). Effective June 1, 2006, coverage will include $300.00 per person, per practitioner, every 12 months for paramedical practitioners (Chiropractor, Physiotherapist, Speech Therapist, Massage Therapist). The changes in deductibles and hearing aide allowance acquisition every 36 months. Reimbursement for prescribed drugs covered benefit levels over the existing plan, including generic prescription drug substitution, unless specifically directed otherwise by the Plan employee’s physician, will be based on the cost of the lowest priced therapeutically equivalent generic version of the drugeffective December 1, unless there is a documented adverse reaction to the generic drug2001.

Vision care In addition to the standard benefits, coverage will include hearing aid allowance (lifetime maximum $500.00 per individual) and will include vision care to a maximum of $200.00 and an eye exam every 24 months and hearing aide allowance acquisition introduce Generic Drug Substitution unless medically indicated otherwise. Effective February 27, 2011 vision care will increase to $225.00 every 36 24 months and effective February 27, 2012 vision care will increase to $275.00 every 24 months. Reimbursement The $200.00 vision care benefit may be used towards the purchase of laser eye surgery. Chiropractic shall be covered to a maximum of $300 per insured annually for prescribed drugs each service. Physiotherapy shall be covered by the Plan to a maximum of $300 per insured annually for each service. Effective February 27, 2011 Chiropractic and Physiotherapy shall be covered to a maximum of $350 per insured annually for each service. Eye exams will be based on the cost paid once every two (2) years to a maximum of the lowest priced therapeutically equivalent generic version $40.00 per employee. Hearing tests will be paid once every two (2) years to a maximum of the drug, unless there $50.00 per employee. Existing provisions for private duty nursing services contained in present extended health care plans will be amended to reflect that this benefit is limited to a documented adverse reaction to the generic drugmaximum of ninety (90) eight-hour shifts in any calendar year.
(c) The Hospital agrees to contribute pay one hundred percent (100% %) of the billed premium towards coverage of eligible employees in the active employ of the Hospital under HOOGLIP in effect as of September 28, 1993 or such other group life insurance plan currently in effect providing effect. Such insurance shall include benefits for accidental death and dismemberment in the balance principal amount equal to the amount of the monthly premium is paid by Group Life Insurance to which the employee through payroll deductionsis entitled.
(d) The Hospital agrees to contribute seventy-five percent (75% %) of the billed premiums towards coverage of eligible employees in the active employ of the Hospital under the Blue Cross #9 Dental Plan in effect as of September 28, 1993 or comparable coverage with another carrier (based on the current ODA fee schedule as it may be updated from time to time); effective November with a one (1) year lag) providing the balance of the monthly premium is paid by the employee through payroll deduction. Effective September 1, 2001 dental 2002, Dental recall including preventative services is every nine (9) months; Blue Cross rider Rider #2 (or equivalent) [complete and partial dentures] at 50/50 co-insurance to $1000 1,000 annual maximum; and Blue Cross rider Rider #4 (or equivalent) [crowns, bridgework, and repairs to same] at 50/50 co-insurance to $1000 1,000 annual maximum. Orthodontic coverage at 50/50 co-insurance with a $1,000 maximum per insured lifetime providing the balance of the monthly premiums are paid by the employee employees through payroll deductiondeductions.
(e) The Hospital will provide equivalent coverage to all employees who retire early and have not yet reached age 65 and who are in receipt of the Hospital's pension plan benefits on the same basis as is provided to active employees for semi-private, extended health care and dental benefits. The Hospital will contribute the same portion towards the billed premiums of these benefits plans as is currently contributed by the Hospital to the billed premiums of active employees.
(f) A copy of all current master policies of the benefits referred to in this Article shall be provided to the Union.
