Medical Assistance Program. The Provider further agrees that if precluded or excluded for cause, either voluntary or involuntary, from the Medical Assistance Program, enrollment in and reimbursement from the AUTOCAT Program shall be prohibited. The Provider further agrees to notify the AUTOCAT Program and the Department of Aging in writing within fifteen (15) calendar days of any misdemeanor or felony conviction relating to a Medicare, Medicaid or Pharmacy Practice offense of the Provider or any of its owners or employees.
