Miscellaneous Service Charges. The Signatories agree that MAWC may implement uniform miscellaneous service charges for its water service as contained in Attachment F to this Stipulation and uniform miscellaneous service charges for its sewer service as contained in Attachment G to this Stipulation.

Miscellaneous Service Charges. The Village is authorized to impose miscellaneous fees, charges or surcharges for costs or violations of this Policy associated with providing Wastewater Service to Customers within the Village. These fees may be set forth and adopted from time to time in a resolution adopted by the Village Council. The Village shall have the right to impose a surcharge for miscellaneous violations, services or actions in accordance with the terms set forth herein. The following is a non-exclusive list of miscellaneous charges the Village may impose in connection with certain actions or Services received:
Miscellaneous Service Charges. I understand and agree Calm and Clear Mind Center charges $30 for the first 20 pages of medical records for their processing, preparation, and printing. I understand each page after the first 20 pages will incur a $0.50 charge per page. I understand that it may take up to 15 business days to obtain my records. I understand that returned checks are subject to a $35 service fee. Any returned checks and associated fees must be resolved before any future appointments can be arranged or scheduled. I understand and agree that if The Center has to pay ANY fees to a third party (i.e. collection agency, attorney, court costs, etc.…) for collection of payments, that based on this agreement, I will be responsible for those payments in addition to any service fees owed to the Center.
