MISHAP INVESTIGATION. In the case of a close call, mishap or mission failure, the Parties agree to provide assistance to each other in the conduct of any investigation. For all NASA mishaps or close calls, Partner agrees to comply with NPR 8621.1, "NASA Procedural Requirements for Mishap and Close Call Reporting, Investigating, and Recordkeeping" and "GRC Mishap Preparedness and Contingency Plan (MPCP)."

NASA and Partner agree to provide assistance to each other in the conduct of any investigation. In the event that Partner processing results in a NASA mishap, Partner agrees to comply with NPR 8621.1, “NASA Procedural Requirements for Mishap and Close Call Reporting, Investigating, and Recordkeeping” and Partner agrees to comply with the requirements in KNPR 8715.3-2, "KSC Safety Procedural Requirements for Partners Operating in Joint-Use Facilities", CHAPTER 6: MISHAPS AND CLOSE CALLS for KSC-Reportable Mishaps and Close Calls.

MISHAP INVESTIGATION. In the event of any mishap resulting in the loss, damage, or destruction to property or facilities used in the performance of this Agreement, the private party agrees to provide technical support for any investigation to assess the cause. Both parties agree that the report will be held confidential to the degree allowed by applicable laws.
MISHAP INVESTIGATION. The FAA and AF agree to cooperate and exchange information on mishaps of mutual interest. Details are to be included in a separate MOA.
