Moda P500 Plan. Effective the month following execution of this agreement, the City will provide Plan P500 group health insurance. The P500 plan has coinsurance (In/Out) of 85% / 60%, a $500 single/$1,500 family deductible and a $1,500 single/$3,000 family out of pocket maximum. The plan includes an alternative care benefit of 20 visits per year for acupuncture and chiropractic care; hearing aid benefit of $4,000 every three years; and a $300 vision allowance per calendar year.

Grantee will continue to maintain an active XXX registration with current information at all times during which it has an active award under this Agreement.
