Monthly Membership Fee. Your account has a Monthly Membership Fee that will be billed to your account each month as a charge, whether or not you use your account, and applied against your available credit like other charges. The Monthly Membership Fee amount is listed in the Fee Schedule. Your payment of the Monthly Membership Fee does not affect our right to close your Account or limit your right to make transactions on your Account.
Monthly Membership Fee. Service Provider shall be charged a monthly membership fee of twenty-nine dollars and ninety- nine cents ($29.99) on a monthly basis if Service Provider chooses to join the Platform once Service Provider has cleared each step of the onboarding process as set forth in Exhibit B. The percentage determining the fee shall be set based on the number of appointments or sessions (collectively, the “Sessions”, and each a “Session”) with the Service Provider performs in any Week on the Platform as set forth below: • Five (5) or fewer Sessions in any week — eighty-two percent (82%) of the Client Payments (as defined below) from such Sessions performed during such week;

3.5 Novel Wellness Centre reserves the right to increase membership fee further in its sole discretion, should municipal rates & taxes be increased by local authorities.
3.6 The member shall have access to and usage of Novel Wellness Centre facilities. In exchange for such access and usage, the member undertakes to pay a discounted monthly membership fee via EFT for a minimum period (the term) as stated in clause 2.
3.7 Novel Wellness Centre is hereby authorized to draw/collect from the above-mentioned account with the above mentioned bank institution (or any bank, building society or branch to which the account may be transferred) the installment above on behalf of Novel Wellness Centre. Monthly Membership Fee. “Monthly Membership Fee” means the monthly fee, which Patient agrees to pay, in the amount as calculated on xxx.xxxxxxxxxx.xxx/xxxxxxxxxx under “Build your plan” for Nashville, TN.
Monthly Membership Fee. The amount of your monthly membership fee (“Membership Fee”) is set by your Circle Leader and specified on their registration page. The Membership Fee is paid initially, and then every 30 days thereafter the intial payment. Payments shall be made through the WomanSpeak online registration form via credit or debit card. Monthly Membership Fee. Membership fees start on the date of enrollment and are automatically billed monthly on the same date each month. Any fees or charges that are not included in the Membership Fee (i.e. fees for noncovered services, such as labs, medications, procedures, etc.) shall be due at the time of service. I understand that I may not prepay the fees associated with this agreement.
a. Adult Individual Membership Fee, for ages 19+: $65.
b. Child Membership Fee, for ages 18 and under: $35 (must have at least one adult member enrolled).
c. Family Membership Fee, including 2 adults and 2 children: $165.
i. Additional children may be added: $20 each child, up to a family maximum of $205.
d. Student Membership Fee, for full-time students only, proof of enrollment required: $45.
Monthly Membership Fee. In addition to the Registration Fee, each Patient shall pay a Monthly Membership Fee (“MMF”) according to the fee schedule noted in Appendix B.
Monthly Membership Fee. Monthly Membership Fees shall be paid by the Principal using a credit/debit card entered into the electronic health record on the date of enrollment. Monthly Membership Fees shall be paid by Principal on the 1st day of every month. The Monthly Membership Fee is pro-rated for the 1sr month of enrollment if the Principal completes enrollment after the 1st day of the month. This fee is not pro-rated in the month of termination. This fee is non-refundable.
