Monthly Membership Fee. Service Provider shall be charged a monthly membership fee of twenty-nine dollars and ninety- nine cents ($29.99) on a monthly basis if Service Provider chooses to join the Platform once Service Provider has cleared each step of the onboarding process as set forth in Exhibit B. The percentage determining the fee shall be set based on the number of appointments or sessions (collectively, the “Sessions”, and each a “Session”) with the Service Provider performs in any Week on the Platform as set forth below: • Five (5) or fewer Sessions in any week — eighty-two percent (82%) of the Client Payments (as defined below) from such Sessions performed during such week;

Monthly Membership Fee. Your account has a Monthly Membership Fee that will be billed to your account each month as a charge, whether or not you use your account, and applied against your available credit like other charges. Your payment of the Monthly Membership Fee does not affect our right to close your Account or limit your right to make transactions on your Account.

In exchange for such access and usage, the member undertakes to pay a discounted monthly membership fee via EFT for a minimum period (the term) as stated in clause 2. 3.7 Novel Wellness Centre is hereby authorized to draw/collect from the above-mentioned account with the above mentioned bank institution (or any bank, building society or branch to which the account may be transferred) the installment above on behalf of Novel Wellness Centre. Monthly Membership Fee. In addition to the Registration Fee, each Patient shall pay a Monthly Membership Fee (“MMF”) according to the fee schedule noted in Appendix B.
