Monthly Periodic Fee Clause Samples
The Monthly Periodic Fee clause establishes the requirement for the regular payment of a specified fee on a monthly basis throughout the term of an agreement. This clause typically outlines the amount due, the due date each month, and the method of payment, applying to ongoing services such as software subscriptions, equipment rentals, or maintenance contracts. Its core function is to ensure predictable, recurring revenue for the service provider while clarifying the payment obligations for the customer, thereby reducing the risk of payment disputes.
Monthly Periodic Fee. (a) Patient will pay a monthly periodic fee (the “Monthly Periodic Fee”) for ongoing Services under the Membership Plan. Services included in the Monthly Periodic Fee will be listed on Practice’s website and are subject to change as set forth herein.
(b) As of the date of this Agreement, the Monthly Periodic Fee is as follows:

(b) As of the date of this Agreement, the Monthly Periodic Fee is as follows:
(i) $249 per month for the Primary Patient (ages 26+);
(ii) $199 per month per additional adult Patient (ages 26+) (if Spouse/ Domestic Partner of Primary Patient);
(iii) $99 per month for the Primary Patient (ages 18-25) and per additional adult Patient (ages 18-25) (if Spouse/Domestic Partner of Primary Patient); and
(iv) $49 per month per additional dependent Patient (ages 18-25).

As of the date of this Agreement, the Monthly Periodic Fee is as follows: $249 per month for the Primary Patient (ages 26+); $199 per month per additional adult Patient (ages 26+) (if Spouse/Domestic Partner of Primary Patient); $99 per month for the Primary Patient (ages 18-25) and per additional adult Patient (ages 18-25) (if Spouse/Domestic Partner of Primary Patient); and $49 per month per additional dependent Patient (ages 18-25). Medicare Patient Agreement (Medicare Patients Only) This MEDICARE PATIENT AGREEMENT (“Agreement”) is entered into by and between ▇▇▇▇▇▇▇ ▇▇▇▇▇▇▇▇, M.D. (the “Physician”), whose principal medical office is located at ▇▇▇ ▇▇▇▇▇▇▇▇▇ ▇▇▇, ▇▇▇▇▇ ▇▇▇ ▇▇▇▇▇ ▇▇▇▇▇ ▇▇▇▇▇, ▇▇ ▇▇▇▇▇, and _______________________________________, a beneficiary enrolled in Medicare Part B (“Beneficiary”), who resides at _______________________________________________________.
Monthly Periodic Fee. This fee is for ongoing primary care services. We prefer that you schedule visits more than 24 hours in advance when possible. We do not provide walk-in urgent care services.
Monthly Periodic Fee. This fee is for monthly ongoing primary care services. There is no copay for office visits. Visits will be included in the monthly fee as long as the number of visits is deemed ‘reasonable’ and on par with the number of annual primary care office visits patients utilize nationwide. Excessive number of office visits will incur an additional $50 per visit charge. Cost for ancillary services will be passed on to Patient. These will be clearly stated in advance of services provided. Monthly fee is due on the first of the month.
