Network Access Services. In lieu of any other rates and discounts, Customer will pay fixed monthly recurring, per-circuit local loop charges ranging from $1,308 to $1,818.57 for DS-1 and DS-3 Access Service at 4 CLLI codes mutually agreed upon by the Customer and Company. Discounts: Data Services: In lieu of any other rates and discounts, Customer will receive a discount of 20% for the following Data Services: Network Access Service: Standard VBSIII local loop charges for DS0, DS-1 and DS-3 Network Services Local Access Services.

Network Access Services. In lieu of any other rates and discounts, Customer will pay a fixed monthly recurring local loop charge of $6,000 and a non-recurring charge of $6,000 and a non-recurring charge of $0 for OC-3 Network Access Service at 1 Circuit ID mutually agreed upon by Customer and Company.

In order to receive the Special Pricing, Customer must maintain the OC-3 circuits for a minimum of 3 years. If Customer terminates the OC-3 before the 3 year minimum Service Term for other than Cause, Customer will pay an amount equal to 100% of the monthly recurring charge for the discontinued OC-3 circuit(s) multiplied by the number of months remaining in the unexpired portion of the 3 year commitment, plus a pro rata portion of any and all credits received by Customer in connection with the OC-3 circuits in addition to any amounts owed for service already received. Company agrees to waive any Circuit Term/Early Term charges for circuits ordered prior to October 1, 2012 that are disconnected after April 30, 2015.

This rate is in lieu of all other rates and discounts and shall remain fixed for the Term. Network Access Services. (A) Aetna shall provide Plan Participants with access to Aetna's network hospitals, physicians and other health care providers ("Network Providers") who have agreed to provide services at agreed upon rates and who are participating in the applicable Aetna network covering the Plan Participants.
