Common use of Notice of Address Change Clause in Contracts

Notice of Address Change. The Subscriber must notify CareFirst BlueChoice within fifteen (15) days of a change in residence or change in e-mail address, if the Member has consented to receive notices via electronic mail, or as soon as reasonably possible. Except in the case of a covered child who does not reside with the Subscriber, CareFirst BlueChoice is only responsible for mailing notices or correspondence to the last known physical address or e-mail address of the Subscriber.
