Notice to supervision. Any employee who requests family leave will, if possible, provide a two (2) week notice to his supervisor prior to the first day of the leave. The employee will also provide the supervisor within ten (10) days advance notification of the date he intends to return to work. Requests for family leave must be made in writing and, where taken for the employee’s or another’s serious illness, must be supported by written medical certification of a health care provider.
