Notification by Employee. When an employee is unable to report to work, he shall notify a Building Administrator or other system as designated by the administration no later than one (1) hour in advance of the time he is scheduled to report to work on each day of absence, unless emergency conditions make it impossible. If the employee is aware of a known illness or injury that will cause frequent or extended absences, the employee is to relate that information to the administration as soon as possible. The administration may require a signed physician's statement attesting to such illness or injury or require that the employee provide a written statement giving the name, address, and phone number of his physician and the dates he consulted with him.
