For Hire Vehicles - For Hire Vehicles shall include all vehicles used for ground transportation of passengers and baggage for compensation, including, but not limited to, taxicab operations, non-exclusive scheduled bus/van/limousine service and non-exclusive prearranged bus/van/limousine/taxi operations. Any Operator that is compensated for transporting passengers or provides transportation as an ancillary service will be considered in this classification.
OPERATOR PRIVILEGES AND OPERATING REQUIREMENTS. A. Operator shall utilize Airport property for the purpose of operating a service under the following category 1. Courtesy Vehicle Operator - Courtesy Vehicle Operator shall include all motor vehicles used for ground transportation of persons and their baggage or personal property, without direct or indirect compensation, remuneration, consideration or exchange of value provided by such person to the Operator, between the Airport and business and/or commercial establishments. Such Operators shall not be for public hire. Any Operator that is compensated for transporting passengers or provides transportation as an ancillary service will not be considered in this classification. B. The Airport grants to the Operator the right to operate and maintain a Courtesy Vehicle Service for transporting passengers and baggage to and from SIA. SAMPLE C. The issuance of an Agreement is subject to the terms and conditions of Resolution No. 04-17, and as may be amended, attached hereto and made a part hereof and any subsequent resolutions adopted or implemented by the Airport as they pertain to Ground Transportation Services at SIA. Operator agrees to abide by all rules, regulations and resolutions adopted by the Airport. D. Operator has acquired and agrees to maintain all federal, state and local licenses and permits required to operate in the City and County of Spokane, Washington necessary to conduct Courtesy Vehicle Services at SIA as outlined on Exhibit A – Ground Transportation Licensing Requirements. Copies of said licenses and permits shall be provided to the Airport prior to entering into an agreement. E. Prior to permitting a person to act as an Operator or Operator’s Driver(s), and annually thereafter, Operator shall obtain a criminal history and background report for such person and provide a copy to the Airport upon request. The criminal history and background report shall consist of both (i) a Washington State Patrol criminal history report and a (ii) Federal Bureau of Investigation fingerprint background check (FBI Identity History Summary) that includes a search of the national sex offender database. Any person who has been convicted, within the past seven years of driving under the influence of drugs or alcohol, or who has been convicted at any time for fraud, sexual offences, use of a motor vehicle to commit a felony, a crime involving property damage, and/or theft, acts of violence, or acts of violence, or acts of terror shall not be permitted to be emplo...

  • Operating Requirements Any operating and technical requirements that may be applicable due to Regional Transmission Organization, Independent System Operator, control area, or the Connecting Transmission Owner’s requirements, including those set forth in the Small Generator Interconnection Agreement. LPHA must operate its Communicable Disease program in accordance with the Requirements and Standards for the Control of Communicable Disease set forth in ORS Chapters 431, 432, 433 and 437 and OAR Chapter 333, Divisions 12, 17, 18, 19 and 24, as such statutes and rules may be amended from time to time. b. LPHA must use all reasonable means to investigate in a timely manner all reports of Reportable Diseases, infections, or conditions. To identify possible sources of infection and to carry out appropriate control measures, the LPHA Administrator shall investigate each report following procedures outlined in OHA’s Investigative Guidelines or other procedures approved by OHA. OHA may provide assistance in these investigations, in accordance with OAR 333-019-0000. As part of its Communicable Disease control program, LPHA must, within its service area, investigate the Outbreaks of Communicable Diseases, institute appropriate Communicable Disease control measures, and submit required information in a timely manner regarding the Outbreak to OHA in Orpheus (or Opera for COVID-19 Cases and XXXXX for COVID-19 contacts) as prescribed in OHA CD Investigative Guidelines available at: d. LPHA must establish and maintain a single telephone number whereby physicians, hospitals, other health care providers, OHA and the public can report Communicable Diseases and Outbreaks to LPHA 24 hours a day, 365 days a year. LPHA may employ an answering service or 911 system, but the ten-digit number must be available to callers from outside the local emergency dispatch area, and LPHA must respond to and investigate reported Communicable Diseases and Outbreaks. e. LPHA must attend Communicable Disease 101 and Communicable Disease 303 training. f. LPHA must attend monthly Orpheus user group meetings or monthly Orpheus training webinars.

  • Training Requirements Grantee shall: A. Authorize and require staff (including volunteers) to attend training, conferences, and meetings as directed by DSHS; B. Appropriately budget funds in order to meet training requirements in a timely manner, and ensure that staff and volunteers are trained as specified in the training requirements listed at xxxxx://xxx.xxxx.xxxxx.xxx/hivstd/training/ and as otherwise specified by DSHS. Grantee shall document that these training requirements are met; and C. Ensure that staff hired for HIV and syphilis testing are trained to perform blood draws within three (3) months of employment.

A Red Hat Enterprise Linux Software Subscription is bundled with the Red Hat CloudForms Software Subscription and the fees for the Red Hat CloudForms Subscription are based on such bundled use. Any use of the Red Hat Enterprise Linux other than to run the Red Hat CloudForms Software is subject to Red Hat’s standard Software Subscription fees for such use.
