Passwords and Other Security Mechanisms. Based on the information provided by the Participant pursuant to Section 4.1 (Identification of Authorized Users), HIO shall issue a user name and password [and/or other security measure] to each Authorized User that shall permit the Authorized User to access the System and use the Services. HIO shall provide each such user name and password [and/or other security measure] to the Participant and the Participant shall be responsible to communicate that information to the appropriate Authorized User. When the Participant removes an individual from its list of Authorized Users, and informs HIO of the change, pursuant to Section 4.1 (Identification of Authorized Users), HIO shall cancel the user name and password [and/or other security measure] of such individual with respect to the Participant, and cancel and de-activate the user name and password [and/or other security measure] of such individual if that individual is as a result of the change no longer an Authorized User of any Participant.43

Redwood MedNet shall issue a user name and password and/or other security measures, as described in the Policies and Procedures, to each Authorized User that shall permit the Authorized User to access the System and use the Services. Redwood MedNet shall provide each such user name and password and/or other security measures to the Participant and the Participant shall be responsible to communicate that information to the appropriate Authorized User. When the Participant informs Redwood MedNet of the removal of any Authorized User, Redwood MedNet shall de-activate the user name and password and/or other security measures of such individual.

HealtHIE Nevada shall issue a user name and password and/or other security measures, as described in the Policies and Procedures, to each Authorized User that shall permit the Authorized User to access the System and use the Services. HealtHIE Nevada shall provide each such user name and password and/or other security measures to the Participant and the Participant shall be responsible to communicate that information to the appropriate Authorized User. When the Participant informs HealtHIE Nevada of the removal of any Authorized User, HealtHIE Nevada shall deactivate the user name and password and/or other security measures of such individual as defined in Policies and Procedures.

Passwords and Other Security Mechanisms. Based on the information provided by Participant for its Authorized Users, HEALTHeLINK shall issue a user identifier and other security measures required by HEALTHeLINK from time to time, to each Authorized User. Upon issuance by HEALTHeLINK of a user identifier and other security measures to the Participant, the Authorized User shall be authorized to access HEALTHeLINK System and use HEALTHeLINK Services consistent with the rights of his or her Participant under its Registration Agreement, the Policies and Procedures, and the Authorized User’s role(s) for the Participant. If HEALTHeLINK provides the user identifier and any other security measure(s) to the Participant for implementation with Participant’s Authorized Users, the Participant shall be responsible for and oversee the implementation and use of the user identifier and any other measures appropriate to the Authorized User. Any violation by an Authorized User of this Agreement, the Participant’s Registration Application, or the Policies and Procedures shall be cause for suspension or termination of the Authorized User’s access to HEALTHeLINK System and use of HEALTHeLINK Services. Participant shall notify HEALTHeLINK of removal of an Authorized User from its list of Authorized Users as required under the Policies and Procedures. Participant shall take such actions as are required by the Policies and Procedures and within its systems and control with respect to such formerly authorized user to terminate the individual’s access to HEALTHeLINK System and HEALTHeLINK Services. Following receipt of notice from the Participant, HEALTHeLINK shall take actions required by the Policies and Procedures and within its systems and control to terminate the individual’s access to HEALTHeLINK System and HEALTHeLINK Services.

(d) Has acknowledged in writing that his or her failure to comply with the Terms and Conditions may result in the withdrawal of privileges to use the Services and the System and may constitute cause for disciplinary action by Participant. Documentation, including the signed Access Request Form, shall be retained by Participant for at least six (6) years and made available to SJCHIE upon request.

Passwords and Other Security Mechanisms. 3.5.1 Upon DBH approval, DBH will issue a unique user ID, password and/or other security measure, as described in the ICANS xXxxxxx, to each Authorized User, which will permit the Authorized User to access the ICANS. DBH restrict ICANS access and/or other security measures based on the information given by the Agency (Identification of Authorized Users) of the ICANS Agency Agreement, and such other information as DBH determines appropriate.
