Past Performance Information for Recent and Relevant Contracts. In accordance with the page limitations set forth in Table 1.1, Offerors shall provide information requested in the Past Performance Information Template, Attachment 5, for each reference. The information provided will be used to help determine relevancy. The Offeror shall provide a formal performance evaluation if available. Formal performance evaluations are those within the Contractor Performance Assessment Reporting System (CPARS) or something similar. If the formal evaluation is available, it shall be included, and the most recent evaluation shall be provided. If a CPARS is included, a Past Performance Questionnaire (PPQ) is not required. If a formal evaluation is unavailable, submit information using the format contained in Attachment 4, Past Performance Questionnaire and Cover Letter. Offerors shall submit Past Performance Information on recent (active within the past three years from the date of this solicitation) Contracts/Task Orders that the Offeror considers most relevant in demonstrating their ability to perform the proposed effort. The total of Past Performance Information references shall not exceed three. References must be for the prime Offeror itself or individual TA members; and at least two must be for the prime Offeror. Information on up to one (1) recent Contract performed by an Offeror’s significant teaming partners and significant subcontractors considered most relevant in demonstrating their ability to perform the proposed effort may be submitted. The Government considers a significant subcontractor and / or teaming partner to be a company who provides 20% or more of the support required, and / or a company who provides a critical portion of the effort required and / or influences the results of performance. References for proposed subcontractors not conisdered signficant are not desired and will not be considered relevant.   • Quarterly Contractor Performance Reporting Customers shall complete a Contractor Performance Survey (Exhibit I) for each Contractor on a Quarterly basis. Customers will electronically submit the completed Contractor Performance Survey(s) to the Department Contract Manager no later than the due date indicated in Contract Exhibit D, Section 17, Additional Special Contract Conditions. The completed Contractor Performance Survey(s) will be used by the Department as a performance-reporting tool to measure the performance of Contractors. The Department reserves the right to modify the Contractor Performance Survey document and introduce additional performance-reporting tools as they are developed, including online tools (e.g. tools within MyFloridaMarketPlace or on the Department's website).
