Past Performance Questionnaire. For all contracts identified in Section 1, Contract Descriptions, a Past Performance Questionnaire (Appendix C) shall be completed and submitted. The Offeror and major subcontractor(s) shall complete Part I of the Past Performance Questionnaire. Once complete the Offeror and major subcontractor(s) shall e-mail the questionnaire to both the Government contracting activity and technical representative responsible for the past/current contract. The POCs for the past/current contract shall be instructed to electronically complete Part II of the questionnaire and e-mail the entire questionnaire to the Contracting Office NLT the proposal due date. The Offeror and major subcontractors shall e-mail the Contracting Officer a list of all the POCs who were sent a questionnaire. The Government shall receive this list NLT the proposal due date. The POC List shall be submitted in MS Word, in Windows Table Format, to include the following fields: Solicitation Number; Company Name; Contract Number; Government Agency; POC Last Name, First Name; POC Title; POC Telephone Number; POC E-mail Address; and Date E-Mail to POC (month/day).

If any negative past performance information is received, the contractor will be given an opportunity to provide rebuttal. A maximum of three (3) Questionnaires must be received by Contract Specialist prior to the date and time established for the RFP closing. The offeror is responsible for ensuring that the Contract Specialist has received a maximum of three (3) questionnaires prior to that date/time.

The Government will focus its inquiries on the Offeror’s (including major subcontractors) record of performance as it relates to the solicitation requirements that the Offeror or major subcontractor is proposed to perform, including cost, schedule, performance and management of subcontractors. Offerors are reminded they must include the most recent and relevant efforts (performance within the past three (3) years) in their proposal. Absent any recent and relevant past performance history, the Offeror will not be evaluated either favorably or unfavorably on past performance.

7. Submissions. Offerors are discouraged from providing points of contact with another contractors’ facility, i.e., in case an Offeror (or one of his/her team members) is in a subcontract with another contractor who has submitted a proposal on the same requirement. Offerors shall provide and submit the prime contract number and all Governmental agency points of contact (POC) in lieu of subcontract numbers or prime contract POCs in situations as described above.
8. Small Business Participation Past Performance All Offerors shall submit information substantiating the Offerors’ past performance in complying with FAR 52.219-8, “Utilization of Small Business”, maximizing opportunities for U.S. small business subcontractors. Offerors shall also provide a statement indicating whether any negative information has been reported in the past six years concerning the Offerors’ past compliance with FAR 52.219-8. If any such negative information has been reported, the Offeror may submit explanations or comments responding to such negative information. Offerors with no prior contracts containing FAR 52.219-8 shall certify the same.
9. It is the sole responsibility of the Offeror to provide their references with a copy of the questionnaire. It is also the Offeror's responsibility to ensure the references return the questionnaire directly to the Contracting Office listed in paragraph three below.

L.3.4.4.1 The Government may utilize past performance information from other sources than those identified by the Offeror, (i.e. Contractor Performance Assessment Reporting System (CPARS)).
L.3.5 Volume V - Small Business Utilization.
L.3.5.1 The Small Business (SB) Utilization Volume is comprised of two distinct elements: the Small Business Participation Plan (Factor) and the Small Business Subcontracting Plan (Not a Factor). The Offeror shall describe the approach to maximizing opportunities for small businesses for this effort, both in terms of the percentage of overall delivery order value (SB Participation Plan) and in terms of the percentage of the subcontracted portion’s value (SB Subcontracting Plan).
