Past Performance Questionnaire. For all contracts identified in Section 1, Contract Descriptions, a Past Performance Questionnaire must be completed and submitted. The Offeror shall complete Part I of the Past Performance Questionnaire and e-mail the questionnaire to both the Government contracting activity and technical representative responsible for the past/current contract. The POCs shall be instructed to electronically complete Part II of the questionnaire and e-mail the entire questionnaire to the Contracting Office no later than the proposal due date, to xxxxx.x.xxxxxxxxx.civ@xxxx.xxx and xxxxxx.x.xxxxxxxxx.civ@xxxx.xxx. Any questionnaires not returned directly to the Contract Specialist will not be reviewed and evaluated. The information contained in the questionnaires will be used to evaluate the offeror’s past performance. If any negative past performance information is received, the contractor will be given an opportunity to provide rebuttal. A maximum of three (3) Questionnaires must be received by Contract Specialist prior to the date and time established for the RFP closing. The offeror is responsible for ensuring that the Contract Specialist has received a maximum of three (3) questionnaires prior to that date/time.
Past Performance Questionnaire. The offeror shall contact past performance references directly and request that they complete the Past Performance Questionnaire (PPQ) (Exhibit B) and submit directly to xxxxx.xxxxxx@xxxx.xxx. Under no circumstances shall the questionnaires be returned to the offerors. Any information provided by the offeror’s references is subject to verification and validation by the Government during the evaluation process. The offeror may provide information on problems encountered on the identified contracts as well as the offeror’s corrective actions.

The Government will focus its inquiries on the offeror’s (including major subcontractors) record of performance as it relates to the solicitation requirements that the Offeror or major subcontractor is proposed to perform, including cost, schedule, performance and management of subcontractors. Offerors are reminded they must include the most recent and relevant efforts (performance within the past nine (9) years) in their proposal. Absent any recent and relevant past performance history or when the performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned, the Offeror will not be evaluated either favorably or unfavorably on past performance.

The Past Performance Questionnaire will not be a part of the resultant contract.
Past Performance Questionnaire. The Offeror is responsible for the distribution, submission, and tracking of the Past Performance Questionnaire (Attachment 6). The Offeror shall exert its best efforts to provide at least two points of contact (Procuring Contracting Officer, Program/Project Manager, Administrative Contracting Officer, or equivalent) per relevant contract, and ensure such information is complete and accurate. Completed Past Performance Questionnaires shall be submitted directly to the Government by the point of contact not later than the due date for the proposal. The Government retains discretion but is not required to consider Past Performance Questionnaires received after the proposal submission due date. Each of the Offeror’s points of contact (POC) shall e-mail the completed Past Performance Questionnaire no later than the RFP closing date to xxxx0@xxxxx.xxx. Once the Past Performance Questionnaires are completed by your POCs, the information contained therein shall be considered sensitive and shall not be released.
