Patient Care Records. PROVIDER personnel will forward directly to AGENCY, when requested, clinical notes and observations for each AGENCY patient served, to be incorporated into the patient's record by AGENCY. The PROVIDER will maintain strict confidentiality of the patients and the nature of any services provided and will exercise the utmost good faith with respect to maintaining the confidentiality of the information and materials learned by virtue of this Agreement.

Patient Care Records. The Ambulance shall complete all PCRs in a timely manner, including all supporting documentation, patient signatures, hospital face sheets, and PCSs for non-emergency transports. The Ambulance also agrees that it will not submit any pre-hospital care report to EMR that violates any New York State or Federal law or regulation governing Social Security, Medicare or Medicaid, and that the U.S. Department of Health, or its designee, and any Payer to which The Ambulance submits claims through EMR may audit The Ambulance, and the records relating to EMR's services for The Ambulance, and to provide such information, as requested, to the requesting entity. EMR acknowledges that all such medical records shall be the property of The Ambulance.

All clinical notes and observations shall be incorporated in the patient's records within 48 hours of the date of service unless otherwise directed by AGENCY staff.

  • Patient Records Upon termination of this Agreement, the New PC shall retain all patient dental records maintained by the New PC or the MSO in the name of the New PC. During the term of this Agreement, and thereafter, the New PC or its designee shall have reasonable access during normal business hours to the New PC's and the MSO's records, including, but not limited to, records of collections, expenses and disbursements as kept by the MSO in performing the MSO's obligations under this Agreement, and the New PC may copy any or all such records.
