Patient Communication. Customer authorizes Athelas to communicate with Customer’s patients on behalf of Customer to the extent necessary to provide the Services. Communication will be via phone, video chat, or electronic means such as text messages, online chats, and emails. If Athelas provides Customer portals for patient review of results, communication may take place via the portal.
Patient Communication. A. On or after the Transfer Date, Custodian may, in its discretion and at its expense, send letters to some or all Patients to whom the Medical Records relate, solely for the purpose of informing them that Custodian now has custody of their Medical Records at the New Practice.
Patient Communication. On-Site Rx will provide consultation to the City with regards to the most effective patient communication strategies to achieve the City’s goals as pertains to an onsite pharmacy and pharmacy services. On-Site Rx also agrees to provide appropriate signage for the pharmacy. The City will be responsible for all costs associated with the printing and distribution of the agreed upon communication materials.
Patient Communication. Notwithstanding anything in this Agreement that could be interpreted as being to the contrary, HOMELINK encourages and expects Participating Provider to communicate freely with patients regarding the treatment options available to them including, without limitation, alternative medications, regardless of benefit coverage.

By providing your email address, phone number, and mailing address, you are giving us permission to contact you through these communication methods, including automated text and voice message systems. You understand that you may opt out of any automated message system at any time. Acknowledgement of Financial Policy: I have read, understand and agree to the above terms and conditions. I authorize my insurance company to pay my health/ medical benefits directly to my doctor’s office.
