Personnel and Accounting. 1. The Organization shall submit a written list of the names, addresses, and phone numbers of all officers, board members, staff working the events and copies of all other operational procedures and expected revenues at least (14) days prior to start of each season.

Personnel and Accounting. The Organization must be a 501(c)(3) not for profit corporation, duly established pursuant to Chapter 617, Florida Statutes, located and operating within Pasco County. The Organization shall submit a written list of the names, addresses, and phone numbers of all officers, board members, yearly operations budget showing all expected revenues and expenditures, the Organizations by-laws and copies of all other operational procedures at least (14) days prior to start of each season. If the 501(c)(3) status is rescinded, suspended, cancelled, or otherwise changed, the Organization shall immediately notify the County. The County may terminate this Agreement immediately. The Organization shall provide an Organization Representative and an Alternative Representative to act as a liaison to the Parks and Recreation Department for the duration of the season. An Organization Representative shall be present at all Organization activities held at the facilities. The Organization shall advise the County in writing within five business days of any changes in Organization Representation (e.g. change in president or contact person). The Organization shall provide significant volunteer leadership and administration resources to ensure capability of complying with the terms of this Agreement. The Organization shall provide a signed and notarized affidavit that provides that background screening for all coaches and volunteers who have direct contact with one or more minors on County property has been performed. Said affidavit shall ensure that all coaches and volunteers are not listed as a Sexual Offender/Predator on the Florida Department of Law Enforcement (FDLE) website at xxx.xxxx.xxxxx.xx.xx and the United States Department of Justice National Sexual Offender website at xxx.xxxxx.xxx pursuant to and in accordance with Section 943.0438, Florida Statutes, at least (14) days prior to the beginning of the respective sports season. Supplemental affidavits shall be required prior to additional coaches or volunteers coming into direct contact with one or more minors on County property during the sports season. No coach, assistant coach, or referee shall commence any sports activities at the County’s facilities until such time the background screening is completed and provided to the County.   • CHIEF EXECUTIVE OFFICER AND CHIEF FINANCIAL OFFICER CERTIFICATION The undersigned Chief Executive Officer and Chief Fiscal Officer of the Recipient, as both are designated in Appendix B of the Agreement, hereby request the Director to disburse financial assistance moneys made available to Project in Appendix C of the Agreement (inclusive of any amendment thereto) to the payee as identified below in the amount so indicated which amount equals the product of the Disbursement Ratio and the dollar value of the attached cost documentation which was properly billed to the Recipient in exclusive connection with the performance of the Project. The undersigned further certify that:

  • Data Controller The Sponsor shall be the data controller for such personal data except that, if Quintiles deals with any personal data under this Agreement in the manner of a data controller, Quintiles shall be the data controller of such personal data to the extent of such dealings.
