Common use of Physician Statement Clause in Contracts

Physician Statement. If medical attention is required, the employee shall be required to furnish a statement from a licensed physician notifying the Engineer that the employee was unable to perform his duties. Where sick leave is requested to care for a member of the immediate family and medical attention is required, the Engineer may require a physician’s certificate to the effect that the presence of the employee is necessary to care for the ill person.

Where sick leave is requested to care for a member of the immediate family and medical attention is requiredfamily, the Engineer may Employer shall require a physician’s certificate if medical attention is required to the effect that the presence of the employee is necessary to care for the ill person.

Where sick leave is requested required to care for a member of the immediate family and medical attention is requiredfamily, the Engineer Employer may require a physician’s certificate medical statement to the effect that the presence of the employee is necessary to care for the ill personperson or in the case of childbirth and other conditions related thereto during the post-natal period that the presence of the employee is necessary to care for the employee's wife and family.
