Extension of Leave. If an extension of the leave is required, a request for the extension must be submitted on the Leave Request Form at least five (5) days in advance of the leave expiration. Consideration of an extension will be based on the same criteria as the original request. Failure to return to work at the expiration of the leave may result in termination.
Extension of Leave. If unable to return at the end of a calendar year, an employee may be placed on some other type of paid or unpaid leave for which he/she meets eligibility requirements.
Extension of Leave. A one-year leave may be extended for an additional year. Application for extension must be submitted in writing, to the superintendent or designee by February 1, and will be granted or denied no later than March 1. If an extension is denied, the teacher has the right to reinstatement pursuant to Section 2 of this Article if the teacher requests reinstatement by April 1.
Extension of Leave. A leave of absence for disability will not be extended beyond 12 months unless there is positive evidence supported by medical opinion that the employee will be able to return to full-time active employment within a reasonable period of time.
Extension of Leave. Upon the exhaustion of protected leave, an employee may request in writing an extension of leave in an unprotected status in accordance with Section 15.2.1. Return from the extended leave will be in accordance with Section 15.2.2.
Extension of Leave a. A less than 12-month employee who desires an extension of leave for a part or all of the following school year must submit a request to the Superintendent in writing by April 1 of the school year for which leave was granted. In the event leave was granted after April 1, written request for an extension of the leave shall be made as soon as feasible, but in no event later than three weeks prior to the employee’s first scheduled duty day in the new school year.
b. A 12-month employee who desires an extension of leave for part or all of the following school year must submit a request to the Superintendent in writing at least one month prior to the termination of the leave.
c. The District shall notify each employee on leave of this provision, by certified mail, to his/her last known address by March 15. The employee shall be required to indicate his/her intent to return, request an extension, or resign. If the employee fails to respond, s/he shall be considered to have resigned effective upon the expiration of the leave.
Extension of Leave. At the end of any child care leave extension beyond one
(1) year, the bargaining unit employee may apply for a vacancy in the same or similar classification, or for any vacancy for which he/she is qualified.
Extension of Leave. An employee who is unable to return after sixty (60) days shall be continued on Industrial Accident Leave but may be paid from accumulated Illness Leave benefits. At the exhaustion of regular Illness Leave, if still unable to return, the employee shall be placed on unpaid Industrial Accident Leave. The total time of all leave benefits provided under this Article, including unpaid Industrial Accident Leave, shall not exceed thirty-six (36) months for any one (1) industrial accident or industrial illness.
Extension of Leave. Employees who are entitled to leave pursuant to Article 21.1 and 21.2 or Article 21.3 shall be entitled to an extended leave of up to an additional six (6) months for health reasons where a doctor's certificate is presented. Such written request must be received by the Employer at least four (4) weeks prior to the expiration of leave taken pursuant to Article 21.1, 21.2 or 21.3.
Extension of Leave. Part-time government service leave may be extended beyond the four (4) year limit.
