Common use of Physician's Certificate Clause in Contracts

Physician's Certificate. After two days of sick leave, the Mayor's Office, or the department head may require a certificate from a physician indicating the need for sick leave taken or the general fitness of the employee to perform his/her work. The initial request for medical certification must be given to the employee in writing, although subsequent requests may be verbal. The contents of the medical certification will be deemed sufficient if it contains the following information:  Identity of the health care provider;  General nature of the health condition;  The date on which the health condition commenced;  The probable duration of the condition;  Whether employee: a. is unable to perform work of any kind; b. is unable to perform any essential functions of the job (including statement of such functions). The City may request a second opinion, at its expense, from a health care provider it designates.
