Common use of Physician's Certificate Clause in Contracts

Physician's Certificate. After two days of sick leave, the Mayor's Office, or the department head may require a certificate from a physician indicating the need for sick leave taken or the general fitness of the employee to perform his/her work. The initial request for medical certification must be given to the employee in writing, although subsequent requests may be verbal.
