Common use of Physician's Certificate Clause in Contracts

Physician's Certificate. After two (2) consecutive days of sick leave, the Mayor's Office or the Department Head may require a certificate from a physician indicating the need for sick leave taken or the general fitness of the Employee to perform his/her work. The initial request for medical certification must be given to the Employee in writing, although subsequent requests may be verbal.
