An employee returning to an intermittent or reduced schedule, or temporarily modified duties shall submit to Health and Safety a physician’s certificate to include: • The date the employee was put under the doctor’s care; • The limitations identified based on the physician’s review of the employee’s job specifications; and • The expected duration of treatment and limitations, if foreseeable.
