Premium Rate Sample Clauses

Premium Rate. A Producer which does not qualify as a "$15 Million Contributor" shall contribute to the Health Plan: (i) four dollars eighty-six and three-tenths cents ($4.863) for each hour worked by or guaranteed an employee by such Producer on or after July 29, 2018 to and including August 3, 2019 under the terms of this Agreement, including "straight time" and "overtime" hours on any day worked; (ii) five dollars sixty-one and three-tenths cents ($5.613) for each hour worked by or guaranteed an employee by such Producer on or after August 4, 2019 to and including August 1, 2020 under the terms of this Agreement, including "straight time" and "overtime" hours on any day worked; and (iii) six dollars thirty-six and three-tenths cents ($6.363) for each hour worked by or guaranteed an employee by such Producer on or after August 2, 2020 to and including July 31, 2021 under the terms of this Agreement, including "straight time" and "overtime" hours on any day worked.

Premium Rate. One and one-half (1½) times the employee’s regular rate.

Premium Rate. The monthly premium rate is set forth on the rate notification provided to the Group. In general, the premium is not pro-rated and there are no partial months of coverage. However, if agreed upon by the parties, if coverage will be issued starting on the 16th calendar day of the month or later, then premium for that month will be pro-rated. If for a Group renewal, or the State of Oregon or any other taxing authority imposes upon SHP any new or additional tax or license fee which is levied upon or measured by premium, by SHP’s gross receipts, or by any portion of either, then SHP may amend this Contract to increase the premium by an amount sufficient to cover all such taxes or license fees rounded to the nearest cent, effective as of the date stated in a notice sent to the Group. The effective date of such a premium increase shall not be earlier than the date of the imposition of such tax or license fee increase. SHP shall also have the right to change the premium as of any date that the extent or nature of the risk under this Contract is changed by amendment to this Contract or by reason of any change mandated by law of regulation.
Premium Rate. (i) A scheduled shift on a Monday through Friday Recognized Holiday. (ii) A scheduled shift on a Saturday Recognized Holiday.
