PREPAID HEALTH BENEFITS. 21.01 The Employer shall provide the following group plans for which participation is compulsory for all eligible Employees: (a) Supplementary Benefits Plan which provides benefits at no less than those in place on February 27, 2015 at Xxxxxxx Calgary inclusive of: (i) Vision care coverage providing for annual eye exams and up to $600 every two (2) calendar years per person for corrective lenses. This shall be inclusive of coverage for elective corrective laser eye surgery. (ii) 80% direct payment provision for medication prescribed by a qualified practitioner. The Short-Term Disability shall become effective on the first working day following the expiry of sick leave credits in the case of absence due to injury or hospitalization. (d) At the Employers’ option, a “Supplementary Unemployment Benefit (SUB) Plan” to supplement an eligible Employee’s Employment Insurance to meet the Employer’s obligation to provide benefit payments to an Employee during the valid health-related period for being absent from work due to pregnancy for which she has provided satisfactory medical proof.
(e) A Group Insurance Plan, inclusive of:
(i) Basic Life Insurance;
(ii) Basic Accidental Death and Dismemberment Insurance;
(iii) Long Term Disability Insurance (income replacement during a qualifying disability equal to 66 2/3% of basic monthly earnings at the Basic Rate of Pay to the established maximum following a 120 working day elimination period).

(a) supplementary health and dental plan benefits commence on the date of hire if the date of hire is the first of the month or for those hired after the first of the month benefits commence the first day of the month following the date of hire into a benefits eligible position; and

PREPAID HEALTH BENEFITS. 21.01 19.01 The Employer shall provide the following group plans for which participation is compulsory for all eligible Employees:

The Short-Term Disability shall become effective on the first 1st working day following the expiry of sick leave credits in the case of absence due to injury or hospitalization. (iv) Long-Term Disability (income replacement during a qualifying disability equal to 66 2/3% of basic monthly earnings to the established maximum following a 120 working day elimination period);
(v) A The Employer's Dental Plan which provides shall provide for the reimbursement of 80% of eligible Basic Services; 50% of eligible Extensive Services Services, [including implants and appliances (appliances to include mouth guards for therapeutic use)], ] and 50% of eligible Orthodontic Services (including coverage for adults) ), in accordance with the current year’s fee guide as set out by the current Benefits AdministratorGreat West Life Usual and Customary Fee Guide. A maximum annual reimbursement of $3000 per insured person per benefit year shall apply to Extensive Services. Orthodontic Services shall be subject to a lifetime maximum reimbursement of $3000 per insured person.
21.02 (a) Where the benefits specified in Article 21.01 19.01 are provided through insurance obtained by the Employer, the administration of such plans shall be subject to and governed by the terms and conditions of the policies or contracts entered into with the underwriters of the plan.
21.03 (b) The parties agree that there shall be no substantive change to any benefits provided by the plan, without agreement between the Employer and the Union, unless such changes are required by legislation.
19.03 The premium costs shall be shared 75% by the Employer and 25% by the Employee.
21.04 19.04 The Employer shall distribute to all Employees brochures and other relevant information concerning the above plans, upon hiring and when there are changes to the plans.
(a) 19.05 The Employer shall provide one (1) copy of each of the plans to the Provincial Office of the United Nurses of Alberta.
(b) The Employer shall advise the United Nurses of Alberta of all premium rate changes pursuant to Article 21.01.
21.06 19.06 Such coverage shall be provided to Regular and Temporary Employees after serving a waiting period of three (3) calendar months except for:
(a) a Part-time Employee whose regularly scheduled hours of work are fewer than 15 hours per week averaged over one (1) complete Cycle of the Shift Schedule; and
(b) a Temporary Employee who is hired to work for a position of less than six (6) months; which Employees are eligible to participate only in Articles 21.01(a19.01(a), 21.01(b19.01(b) and Article 21.01(c)(v19.01(c)(v) above.
19.07 The Employer shall advise the Employees covered by benefits of all premium rate changes. This shall be provided in writing as soon as practicable after the Employer is notified of it.
19.08 Providing the Employee is actively at work:
(a) supplementary health and dental plan benefits commence on the date of hire if the date of hire is the first of the month or for those hired after the first of the month benefits commence the first day of the month following the date of hire into a benefits eligible position; and

(b) Alberta Health Care Insurance Plan;
(c) A Benefits Plan Benefit Plan, inclusive of:
(i) Group Life Insurance (1X basic annual earnings rounded to next highest one thousand dollars [$1,000]);

A up to a maximum annual reimbursement of $3000 per insured person per benefit year shall apply to Extensive Services. year; and
(B) 50% of eligible Orthodontic Services shall be subject up to a lifetime maximum reimbursement of $3000 per insured personperson per lifetime.
21.02 19.02 Where the benefits specified in Article 21.01 19.01 are provided through insurance obtained by the Employer, the administration of such plans shall be subject to and governed by the terms and conditions of the policies or contracts entered into with the underwriters of the plan.
21.03 19.03 The premium costs shall be shared 75% by the Employer and 25% by the Employee, except that:
(a) For Part Time or Temporary Employees whose regularly scheduled hours of work are at least 15.5 hours and less than 25 hours per week averaged over one cycle of the shift schedule, the premium costs shall be shared 50% by the Employer and 50% by the Employee.
21.04 (b) For Temporary Employees who are hired to work for a position of more than six months, the premium costs shall be shared pursuant to article 19.03.
19.04 The Employer shall distribute to all Employees brochures and other relevant information concerning the above plans, upon hiring and when there are changes to the plans.
(a) The Employer shall provide one copy of each of the plans to the Provincial Office of the United Nurses of Alberta.
(b) The Employer shall advise the United Nurses of Alberta of all premium rate changes pursuant to Article 21.0119.01.
21.06 19.06 Such coverage shall be provided to Regular and Temporary Employees after serving a waiting period of three (3) calendar months except for:
(a) a Part-time Employee whose regularly scheduled hours of work are fewer than 15 hours per week averaged over one complete Cycle of the Shift Schedule; and
(b) a Temporary Employee who is hired to work for a position of less than six months; which Employees are eligible to participate only in Articles 21.01(a), 21.01(b) and Article 21.01(c)(v) above.than

PREPAID HEALTH BENEFITS. 21.01 The Employer shall provide the following group plans for which participation is compulsory for all eligible Employees:
(a) A Supplementary Benefits Plan or equivalent, which provides benefits at no less than those in place on February 27August 1, 2015 at Xxxxxxx Calgary inclusive of:
(i) Vision vision care coverage providing for annual eye exams and up to $600 every two (2) calendar years per person for corrective lenses. This shall be inclusive of coverage for elective corrective laser eye surgery.; and
(ii) 80% direct payment provision for medication prescribed by a qualified practitioner. (Subject to continuation of Joint Appeal Panel and criteria that medication must be prescribed to correct or treat a medical condition based on a diagnosis made by a physician, dentist or nurse practitioner.)
(iii) The following items will be included in the Supplementary Health Care Plan: • Hearing Aids – Increase to $3000/five years • Diabetic Supplies – provide 100% Direct Xxxx coverage • Diabetic Supplies – Include Insulin Pumps (100% coverage every five years) • Surgical Stockings/Compression Hose – two pair/year • Foot Orthotics - $500/two years • Increase maximum for Psychologist from $50/visit to $100/visit with a maximum of 20 visits per year. • Chiropractic coverage – provide payment under the benefit plan before provincial maximum reached. • Lowest Cost Alternative Medication Coverage, unless otherwise specified • 100% Coverage for respiratory equipment (including CPAP machines and supplies) • Coverage for listed vaccines (Hep A and Hep B)
(b) Alberta Health Care Insurance Plan;
(c) A Benefits Plan Benefit Plan, inclusive of:
(i) Group Life Insurance (1X basic annual earnings rounded to next highest one thousand dollars [$1,000]);
(ii) Accidental Death and Dismemberment (basic) (1X basic annual earnings rounded to next highest one thousand dollars [$1,000]);
(iii) Short-Term Disability (income replacement for a period of up to 120 one hundred and twenty [120] working days during a qualifying disability equal to sixty-six and two-thirds percent [66 2/3% %] of basic weekly earnings to the established maximum following a 14 fourteen [14] day elimination period where applicable. The Short-Term Disability shall become effective on the first (1st) working day following the expiry of sick leave credits in the case of absence due to injury or hospitalization. In the particular case of Employees who have insufficient sick leave credits to satisfy the 14 fourteen [14] calendar day elimination period, the Short- Short-Term Disability shall commence on the 15th fifteenth [15th] day following the commencement of non-hospitalized sickness);
(iv) Long-Term Disability (income replacement during a qualifying disability equal to 66 2/3% of basic monthly earnings to the established maximum following a 120 working day elimination period);
(v) A Dental Plan which provides for the reimbursement of 80% of eligible Basic Services; 50% of eligible Extensive Services [including implants and appliances (appliances to include mouth guards for therapeutic use)], and 50% of eligible Orthodontic Services (including coverage for adults) in accordance with the current year’s fee guide as set out by the current Benefits Administrator. A maximum annual reimbursement of $3000 per insured person per benefit year shall apply to Extensive Services. Orthodontic Services shall be subject to a lifetime maximum reimbursement of $3000 per insured person.
