PREPAID HEALTH BENEFITS. 21.01 The Employer shall provide the following group plans for which participation is compulsory for all eligible Employees: (a) Supplementary Benefits Plan which provides benefits at no less than those in place on February 27, 2015 at Xxxxxxx Calgary inclusive of: (i) Vision care coverage providing for annual eye exams and up to $600 every two (2) calendar years per person for corrective lenses. This shall be inclusive of coverage for elective corrective laser eye surgery. (ii) 80% direct payment provision for medication prescribed by a qualified practitioner. (b) Alberta Health Care Insurance Plan; (c) A Benefits Plan inclusive of: (i) Group Life Insurance (1X basic annual earnings rounded to next highest $1,000); (ii) Accidental Death and Dismemberment (basic) (1X basic annual earnings rounded to next highest $1,000); (iii) Short-Term Disability (income replacement for a period of up to 120 working days during a qualifying disability equal to 66 2/3% of basic weekly earnings to the established maximum following a 14 day elimination period where applicable. The Short-Term Disability shall become effective on the first working day following the expiry of sick leave credits in the case of absence due to injury or hospitalization. In the particular case of Employees who have insufficient sick leave credits to satisfy the 14 calendar day elimination period, the Short- Term Disability shall commence on the 15th day following the commencement of non-hospitalized sickness); (iv) Long-Term Disability (income replacement during a qualifying disability equal to 66 2/3% of basic monthly earnings to the established maximum following a 120 working day elimination period); (v) A Dental Plan which provides for the reimbursement of 80% of eligible Basic Services; 50% of eligible Extensive Services [including implants and appliances (appliances to include mouth guards for therapeutic use)], and 50% of eligible Orthodontic Services (including coverage for adults) in accordance with the current year’s fee guide as set out by the current Benefits Administrator. A maximum annual reimbursement of $3000 per insured person per benefit year shall apply to Extensive Services. PREPAID HEALTH BENEFITS. ‌ 31.01 When the enrolment and other requirements of the insurer(s) as indicated in the contracts with the insurers have been met, the Employer shall implement the following group plans in accordance with the Alberta Health Services Standard Plan: (a) Alberta Health & Wellness Insurance Plan; (b) A Supplementary Health Plan; (c) A Dental Plan, which provides for the reimbursement of at least eighty percent (80%) of eligible basic services; fifty percent (50%) of eligible Extensive Services and fifty percent (50%) of eligible Orthodontic Services, in accordance with the current Alberta Blue Cross Dental Schedule. A maximum annual reimbursement of three thousand dollars ($3,000) per insured person per benefit year shall apply to combined Basic and Extensive Services. Orthodontic Services shall be subject to a lifetime maximum reimbursement of three thousand dollars ($3,000) per insured person;

PREPAID HEALTH BENEFITS. 31.01 The Employer shall contract for and provide the following group plans as outlined in this Article for eligible and participating Full-time and Part- time Employees in accordance with the provisions of the plans and as subject to the requirements of the Insurers: - Extended Medical Benefit Plan, - Dental Plan, - Long Term Disability Insurance Plan, - Life Insurance: Optional Group Life Basic Group Life Dependent Life Accidental Death and Dismemberment (AD & D) Occupational Accident 31.02 The implementation and operation of the Health Boards Association Services (HBA) Benefit Plan, or the equivalent, hereinbefore referred to, shall, at all times, be subject to and governed by the terms and conditions of the policies and contracts entered into with the underwriters of the plans. 31.03 The Employer shall make available to eligible Employees brochures outlining the above plans. 31.04 The HBA, on behalf of all Employer hospitals, will provide one (1) copy of each of the plans to the Provincial Office of the Alberta Union of Provincial Employees. Where the HBA Plan is not in force in any given hospital, that hospital will provide a copy of its plan to the Union.

A maximum annual reimbursement of twenty-five hundred dollars ($2,500) per insured person per benefit year shall apply to combined Basic and Extensive Services. Orthodontic Services shall be subject to a lifetime maximum reimbursement of twenty-five hundred dollars ($2,500) per insured person; (d) Group Life Insurance (e) Basic Accidental Death and Dismemberment (f) Effective April 1, 2006, Short-Term Disability (g) Long Term Disability 34.02 The premiums for the plans outlined in Clause 34.01 will be cost shared, seventy-five percent (75%) by the Employer and twenty-five percent (25%) by the Employee. 34.03 The administration of benefits specified in Clause 34.01 shall, at all times, be subject to and governed by the terms and conditions of the policies and contracts entered into with the underwriters of the Plans. 34.04 The Employer shall make available to eligible Employees brochures outlining the above Plans. (ii) Accidental Death & Dismemberment (Basic). (iii) Short-term Disability (income replacement for a period up to one hundred and twenty (120) working days during a qualifying disability) equal to sixty-six and two thirds percent (66 2/3%) of basic weekly earnings to the established maximum following a fourteen (14) day elimination period where applicable. (iv) Long-term Disability (income replacement during a qualifying disability) equal to sixty-six and two thirds percent (66 2/3%) of basic monthly earnings to the established maximum following a one hundred and twenty (120) working day elimination period. (v) Dental Plan which provides for the reimbursement of eighty percent (80%) of eligible basic services, fifty percent (50%) of eligible extensive services (including implants) and fifty percent (50%) of eligible orthodontic services, in accordance with the current applicable fee guide. A maximum of three thousand dollars ($3,000) per person per benefit year shall apply to extensive services. Orthodontic services shall be subject to a lifetime maximum of two thousand dollars ($2,000) per insured person. (vi) Supplementary Benefit Plan; and (1) Hearing Aids – $3000/five years;0 (2) Foot Orthotics - $500/two years; (3) Psychologist - $100/visit with a maximum of 20 visits per year; (4) Chiropractic coverage – provide payment under the benefit plan before provincial maximum reached; (5) Lowest Cost Alternative Medication Coverage, unless otherwise specified; (6) Coverage for listed vaccines (Hep A and Hep B); (7) Physiotherapy coverage – $50/visit with a maximum of 20 visits per year. (8) Massage Therapy coverage - $50/visit with a maximum of 20 visits per year. (9) Out of Country Travel coverage - $2,000,000 (vii) Vision Care Plan (1) providing for $600.00 every two (2) years per eligible person, including laser eye surgery; and Intraocular Toric contact lenses. (c) All Regular Employees who are regularly scheduled to work at least fifteen (15) hours per week averaged over the course of the shift cycle will be eligible to participate in the benefit plan. PREPAID HEALTH BENEFITS. 27.01 The Employer agrees to provide benefits to eligible Regular Full-Time and Regular Part-Time Employees of The Salvation Army Edmonton Xxxxx Xxxxx in accordance with the terms of the Employer’s Benefit Plan. Employees who regularly work twenty-five (25) hours per week or more shall participate in the benefit plan. Employees who work thirty (30) hours per week or more are eligible for long-term disability. As of the date of ratification, the Benefit Plan includes coverage for: • health care • dental care • long-term disability • life insurance • accidental death and dismemberment. The types of coverage and benefits in the Benefit Plan and the operation of the Benefit Plan shall be governed by the terms and conditions of the Benefit Plan and the contracts between The Salvation Army and the benefit insurers, as provided for in the Employer’s Policy. The Employer shall make information booklets available to eligible Employees who participate in the benefit plans. 27.02 Employees will be given information about the Employee’s premium costs when they are selecting the types of coverage they wish to have under the Benefit Plan. The Employer shall pay one hundred percent (100%) of the premiums for an eligible Employee who selects single coverage for the core plan for health care, dental care, life insurance and accidental death and dismemberment. An eligible Employee is responsible for one hundred percent (100%) of the premiums associated with long-term disability. Employees are responsible for additional premiums associated with couple, family coverage and other options offered under the Benefit Plan. 27.03 In the event the Provincial Government implements the Alberta Health Care Insurance Plan, the Employer shall provide regular Full-Time Employees a benefit equal to the single premium.

PREPAID HEALTH BENEFITS. An Employee who is laid off shall make prior arrangements to pay the full premium of any applicable benefit plans to assure continuation of such protection if so desired. Such arrangements shall continue so long as the Employee has rights to recall.
