Preventive Measures Claim Form. Claims for reimbursement of Preventive Measures Claims must be made by completing and submitting a Preventive Measures Claim Form by no later than the Preventive Measures Claims Deadline (as defined below). The Preventive Measures Claim Form shall be substantially in the form attached as Exhibit 7 to the Settlement Agreement.
