Claim Forms definition

Claim Forms mean the form Class Members (including Minor Subclass Members) must timely submit to receive payment under this Settlement Agreement. The Claim Form must be substantially similar to the form attached as Exhibit 5.

More Definitions of Claim Forms

Claim Forms means Claim Form–A, Claim Form-B, Claim Form-C, and Claim Form-D.

For online purchases, the place of purchase is the Settlement Class Member’s state of residence at the time of the purchase. Additionally, for a Claim to be considered valid, each Settlement Class Member must provide “proof of purchase” concurrently with the Claim Form by submitting one (1) of the following:
Claim Forms refers collectively to the Premium Overpayment Claim Form and Identity Theft Claim Form.
