Procedures for Withdrawal. (The employer will provide each employee with the procedures for withdrawals of contributions received by the financial institution selected by that employee, and that financial institution's name and address (by attaching that information or inserting it in the space below) unless: (1) that financial institution's procedures are unavailable, or (2) that financial institution provides the procedures directly to the employee. See Employee Notification section in the instructions.
Procedures for Withdrawal a. A member requesting days from the Sick Leave Bank shall complete the application form with supporting evidence from the member’s physician, and forward same to the Sick Leave Bank Committee. The Sick Leave Bank Committee shall have the responsibility to either grant, or deny the withdrawal request. If the member is incapacitated and cannot make application for withdrawal, the Committee may make an application on the behalf of an incapacitated member and may grant the withdrawal on his/her behalf.
b. During the period of sick leave bank withdrawal, the member or his/her physician shall submit bi-weekly certifications of continued illness.

Procedures for Withdrawal. A. Withdrawal of Sonera ADSs Tendered Through DTC. The U.S. Exchange Agent will make available the withdrawal procedures established by the DTC Book-Entry Transfer Facility by which Sonera ADSs previously tendered via the DTC Book-Entry Transfer Facility to the U.S. Exchange Agent by an Agent's Message may be properly withdrawn at any time on or prior to the Expiration Date of the Exchange Offer.
B. Withdrawal of Sonera ADSs Evidenced by Sonera ADR Certificates. The U.S. Exchange Agent will establish procedures enabling tendering holders of Sonera ADSs evidenced by Sonera ADR certificates to withdraw such tender by timely delivering a signed notice of withdrawal, guaranteed by an Eligible Guarantor, to the U.S. Exchange Agent at the address in New York set forth in the back cover to the Exchange Offer Prospectus.
Procedures for Withdrawal. Any employee shall become eligible to apply for a withdrawal of days from the Sick Leave Bank after he/she has exhausted all their personal sick leave days. A waiting period of ten (10) days must transpire before Sick Leave Bank days become available.

Procedures for Withdrawal. The withdrawal of a member municipality shall be effected by an amendment to this agreement adopted as provided in this Section IX. The provisions of Sections VII and VIII, relating to all other amendments and to the admission of new member municipalities shall not apply to an amendment under this Section IX. Any member municipality may seek to withdraw from the District by adopting a majority vote to that effect at a meeting of its Legislative Body. After such a vote the Executive Body of said municipality shall notify the District Board of Representatives and request that the District Board of Representatives prepare an amendment providing for its withdrawal. The request shall be accompanied by a certified copy of the vote of the Legislative Body and shall include an agreement by said municipality to pay all of the District's expenses in preparing the requested amendment, whether or not it is finally adopted. Municipalities may request to withdraw from the District at any time. However, notification of withdrawal effective in the next fiscal year must be received by the District no later than the second week of January of the current fiscal year. Requests for withdrawal received after this date shall result in the District requiring full payment of the withdrawing municipality’s assessment for the next fiscal year. Thereupon, and within ninety days from the receipt of the notice and request, the District Board of Representatives shall prepare an amendment providing for the withdrawal of the requesting municipality setting forth such terms as the District Board of Representatives shall deem June 1, 2001
Procedures for Withdrawal or Termination of the FPA
1. Any Project Signatory that wants to terminate or withdraw from the Project is expected to provide written notice to the other parties at least sixty (60) days before the withdrawal or termination.
2. If requested by any Project Signatory during the sixty (60) day period noted above, the dispute resolution proceedings described in this Agreement may be initiated to resolve any dispute relating to the intended withdrawal or termination. If, following any dispute resolution or informal discussion, a Project Signatory still desires to withdraw or terminate, that Project Signatory will provide written notice of final withdrawal or termination to the other Project Signatories. If any agency withdraws or terminates its participation in the Agreement, the remaining agencies will consult with Autoliv to determine whether the Agreement should be continued in modified form, consistent with applicable federal or state law, or whether it should be terminated.
3. The procedures described in this Section apply only to the decision to withdraw or terminate participation in this Agreement. Procedures to be used in modifying or rescinding any rules, permits, or orders implementing this XL project will be governed by applicable law.
