Withdrawal Procedures. Upon the withdrawal of the privileges of this Agreement, the Resident shall vacate the Room within the time frame given, and deliver to the Manager vacant possession of the Room together with all of the furnishings, fixtures, appliances and telephone of the Residence, and the furnishings and fixtures shall be in good condition, with reasonable wear and tear excepted. The Resident will follow the Move-out Procedures explained in section 3.02 of this Agreement and all additional directions communicated by the Manager.
Withdrawal Procedures. Participating members who are currently full-time employees and who are forced into an extended absence from employment because of his/her own catastrophic illness or accident (excluding workers compensation cases) are eligible. The following criteria must be met when applying to the Sick Leave Bank committee established by the Union for withdrawal of days from the Sick Leave Bank.
A. Any employee requesting withdrawal from the Sick Leave Bank who is forced into and extended absence from employment because of his/her catastrophic illness or accident (excluding workers compensation cases) will file an application on the proper form accompanied by a physician's statement which shall include the number of days the employee must be absent from work and the nature of the illness or injury.
B. Any employee requesting withdrawal from the Sick Leave Bank to participate in a rehabilitation program or counseling must provide documentation of enrollment in said rehabilitation program or counseling and any other materials requested.
C. Exhaustion of all personal sick leave days and any other form of sick leave such as, but not limited to, illness-in-the-line-of-duty.
D. Has been absent without pay for at least six (6) consecutive working days as a consequence of the illness, accident, or disability.
E. Be absent from work for at least seven (7) days, six (6) days of which shall not be eligible for compensation under (c). The six (6) day waiting period may be paid days by donated leave days from other employees if the employee so chooses to accept the donation of days subject to the guidelines for leave transfer.
F. Application for use of the Bank for elective surgery shall not be considered. The Sick Leave Bank Committee may require a second opinion from a physician if necessary to determine whether or not surgery is elective, such second opinion will be obtained at the employee's expense.
Withdrawal Procedures. 19.1. In order, to proceed with the withdrawal request the client must login to Client Portal using the following link xxxxx://xx.xxxxxxxxxxxx.xxx/en/Account/LogOn?ReturnUrl=%2f
19.2. Once login successful, the client need to click on Fund/Withdrawal button to proceed with the withdrawal request, as per trading account selected.
19.3. Once the trading account selected, the client must choose the payment method, for the withdrawal to be executed by the company.
19.4. Client must provide all KYC (Know-Your-Client) documents, as per company Compliance and AML requirements, in order to be verified and avoid any fraud activities to protect client funds and the company.
19.5. In case of any issues with the withdrawal request, the client must inform a company by sending an email to xxxxxxx@xxxxxxxxxxxx.xxx
Withdrawal Procedures. A Participant may withdraw all but not less than all the Contributions credited to his or her account and not yet used to exercise his or her option under this Plan at any time by (a) submitting to the Company’s stock administration office (or its designee) a written notice of withdrawal in the form determined by the Administrator for such purpose (which may be similar to the form attached hereto as Exhibit B), or (b) following an electronic or other withdrawal procedure determined by the Administrator. The Administrator may set forth a deadline of when a withdrawal must occur to be effective prior to a given Exercise Date in accordance with policies it may approve from time to time. All of the Participant’s Contributions credited to his or her account will be paid to such Participant as soon as administratively practicable after receipt of notice of withdrawal and such Participant’s option for the Offering Period will be automatically terminated, and no further Contributions for the purchase of shares of Common Stock will be made for such Offering Period. If a Participant withdraws from an Offering Period, Contributions will not resume at the beginning of the succeeding Offering Period, unless the Participant re-enrolls in this Plan in accordance with the provisions of Section 7.
Withdrawal Procedures. Notice of withdrawal should be made in person by submitting a Withdrawal Form to the Registrar, and the date of determination will be the Withdrawal Date the student submits the Withdrawal Form. The Withdrawal Form and procedure may be obtained from HCI’s Registrar in Suite 205 or on HCI’s website: xxx.xxx.
Withdrawal Procedures. An employee may withdraw sick leave from the CLB when all of the following requirements are met:
o A physician certifies that the applicant or immediate family member is disabled by illness or injury o If the applicant is disabled, the physician certifies that he/she is unable to perform the essential duties of his/her classification o If an immediate family member is disabled, the physician certifies that home care by the applicant is necessary o The physician certifies that the disability is expected to continue for more than thirty
Withdrawal Procedures a. The maximum number of days that a member may withdraw is ten percent (10%) of the total days in the bank at the end of the enrollment period (September 15th).
b. The member who borrows days will pay back the days at the rate of fifty percent (50%) of his/her monthly accumulated sick leave until the total number of days borrowed has been restored to the bank. Provided in the event a member who owes days to the bank ceases for any reason to earn sick leave days (i.e., retirement, resignation, permanent disability, or death) any days of sick leave at that time to the credit of such member shall be used to repay the bank before any sick leave days are cashed in for severance pay purposes.
c. SLB may not be used as a means of increasing retirement compensation or severance.
Withdrawal Procedures. 1. The maximum number of times a person may withdraw from the Pool is two times per incident, with 30 days the maximum per request. Sixty (60) days maximum per incident.
2. Withdrawals of sick leave from the S.L.P. shall not require any payback by the acquiring member.
Withdrawal Procedures. A student may withdraw from a course of study at any time by advising the Registrar or Principal in writing on a Withdrawal Form of their intention to do so. No refund of fees will be payable to the student except as stated in the Refund Conditions above. Following withdrawal by a student from their course of study or termination of the student’s enrolment, the College will do the following: If applicable, provide to the student a transcript showing progress made. Where a NZ citizen or permanent resident receives a student allowance, notify Study Link of the withdrawal.
Withdrawal Procedures. 1. FGC Career and Workforce Development Dual Enrollment students are responsible for officially withdrawing from classes they are no longer attending in accordance with published Florida Gateway College requirements and deadlines, e.g. drop/add periods.
2. Students who do not officially withdraw from a class may receive a failing grade.
3. Career and Workforce Development Dual Enrollment students are responsible for notifying the high school guidance counselor, in writing, prior to withdrawal from any Career and Workforce Development Dual Enrollment course.
4. Pre-registration advising, including but not limited to posted withdrawal procedures, is the responsibility of the high schools in the District.
