Records Custodian. The City allows the physical custody of City records by the Port, and the Port allows the physical custody of Port records by the City, for the implementation of this Chapter.
Records Custodian. The Justice of the Peace or County Judge representative who requests services under this agreement is the custodian of the records generated by the ME Office in providing these services.
Records Custodian. MHS will serve as the legal custodian of records solely for purposes of (1) release of Patient Medical Records in accordance with Legal Requirements, and (2) maintaining an accounting of disclosures of Patient Medical Records under HIPAA, in both cases as further described herein.
Records Custodian. The postmaster or other head of a facility such as a postal data center, mailbag depository, management sectional center, district office, or regional headquarters who maintains USPS records. Department heads are the custodians of records maintained at Headquarters. Senior medical personnel are the custodians of restricted medical records maintained within Postal facilities. PAR counselors are the custodians of records pertaining to program participants.
