Referrals to XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. The DORS counselor will encourage DORS consumers, and family members/advocates, as appropriate, to meet with XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. staff at the DORS office or XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. in order to gain a better understanding of XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. services being considered. · The DORS counselor shall provide medical, psychological, social, educational, and other relevant (such as DORS Individualized Plan for Employment) and required information to THE XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. including the specific required referral form for Career Assessment Services and other referral forms as applicable. · XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. has the option of not accepting a referral if it is deemed by XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. that it has not obtained adequate and/or current information or, if in the view of XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. , the person cannot be served by XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. · A DORS Authorization identifying services to be provided, dates of service and cost will be sent to XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. prior to a person entering a service program. · By referring a person for vocational services and entering into a purchase of service agreement, DORS expressly is not entering into an employer-employee relationship with the client or XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. DORS to provide the following services. XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. is responsible for the monthly reporting of progress and, when appropriate, submitting recommendations on persons referred by DORS according to the standards and format indicated in this section. Reports require the signature of the appropriate XXXXXXX XXXXXXX EDUCATION AND COMMUNITY SERVICES, INC. staff member and shall be submitted within the specified timeline.

  • Health Promotion and Health Education Both parties to this Agreement recognize the value and importance of health promotion and health education programs. Such programs can assist employees and their dependents to maintain and enhance their health, and to make appropriate use of the health care system. To work toward these goals:

  • Availability of Verizon Telecommunications Services 3.1 Verizon will provide a Verizon Telecommunications Service to Z-Tel for resale pursuant to this Attachment where and to the same extent, but only where and to the same extent, that such Verizon Telecommunications Service is provided to Verizon’s Customers. 3.2 Except as otherwise required by Applicable Law, subject to Section 3.1, Verizon shall have the right to add, modify, grandfather, discontinue or withdraw, Verizon Telecommunications Services at any time, without the consent of Z-Tel. 3.3 To the extent required by Applicable Law, the Verizon Telecommunications Services to be provided to Z-Tel for resale pursuant to this Attachment will include a Verizon Telecommunications Service customer-specific contract service arrangement (“CSA”) (such as a customer specific pricing arrangement or individual case based pricing arrangement) that Verizon is providing to a Verizon Customer at the time the CSA is requested by Z-Tel.

  • General Education University Program Requirements All MTA applicable courses require a grade “C” or higher

  • Pharmacy Services The Contractor shall establish a network of pharmacies. The Contractor or its PBM must provide at least two (2) pharmacy providers within thirty (30) miles or thirty (30) minutes from a member’s residence in each county, as well as at least two (2) durable medical equipment providers in each county or contiguous county.

  • Health Care The Company will reimburse the Executive for the cost of maintaining continuing health coverage under COBRA for a period of no more than 12 months following the date of termination, less the amount the Executive is expected to pay as a regular employee premium for such coverage. Such reimbursements will cease if the Executive becomes eligible for similar coverage under another benefit plan.

  • Agreement Administration and Communications A. Under this Agreement, either of the representatives of the Judicial Council identified below will monitor the Work and act as the Judicial Council’s liaisons with the Contractor: i. The Judicial Council may reallocate funds between the estimated amounts set forth in Exhibit G, without an Amendment to this Agreement, as long as the total amount to be paid under this Agreement does not exceed the Contract Amount.   • In-Service Education The parties recognize the value of in-service both to the employee and the Employer and shall encourage employees to participate in in-service. All employees scheduled by the Employer to attend in-service seminars shall receive regular wages.

  • FUND ADMINISTRATION SERVICES BNY Mellon shall provide the following fund administration services for each Fund, Series and class:  Calculate Fund approved income and per share amounts required for periodic distributions to be made by the applicable Fund, Series or class;  Coordinate a Fund’s annual audit and respond timely and completely to related requests;  Cooperate with each Fund’s independent auditors;  Supply various normal and customary portfolio and Fund statistical data as requested on an ongoing basis; and  If the chief executive officer or chief financial officer of a Fund is required to provide a certification as part of the Fund’s Form N-Q or Form N-CSR filing pursuant to regulations promulgated by the SEC under Section 302 of the Xxxxxxxx-Xxxxx Act of 2002, provide a sub-certification in support of certain matters set forth in the aforementioned certification. Such sub-certification is to be in such form and relating to such matters as reasonably agreed to by BNY Mellon in advance. BNY Mellon shall be required to provide the sub-certification only during the term of this Agreement with respect to the applicable Fund or Series and only if it receives such cooperation as it may request to perform its investigations with respect to the sub-certification. For clarity, the sub-certification is not itself a certification under the Xxxxxxxx-Xxxxx Act of 2002 or under any other law, rule or regulation. BNY Mellon shall provide the following regulatory administration services for each Fund and Series:  Assist the Fund in responding to SEC examination requests by providing requested documents in the possession of BNY Mellon that are on the SEC examination request list and by making employees responsible for providing services available to regulatory authorities having jurisdiction over the performance of such services as may be required or reasonably requested by such regulatory authorities;  Assist with and/or coordinate such other filings, notices and regulatory matters and other due diligence requests or requests for proposal on such terms and conditions as BNY Mellon and the applicable Fund on behalf of itself and its Series may mutually agree upon in writing from time to time; and

  • Clinical Management for Behavioral Health Services (CMBHS) System 1. request access to CMBHS via the CMBHS Helpline at (000) 000-0000. 2. use the CMBHS time frames specified by System Agency. 3. use System Agency-specified functionality of the CMBHS in its entirety. 4. submit all bills and reports to System Agency through the CMBHS, unless otherwise instructed.
