Report Contents. To the extent such information is available Business Associate’s report will at least:
(i) Identify the nature of the non-permitted or prohibited access, Use or Disclosure, including the date of the Breach and the date of discovery of the Breach;
(ii) Identify the PHI accessed, used or disclosed, and provide an exact copy or replication of the PHI, as appropriate, in a format reasonably requested by Company, and to the extent available;
(iii) Identify who caused the Breach and who received the PHI;
(iv) Identify what corrective action Business Associate took or will take to prevent further Breaches;
(v) Identify what Business Associate did or will do to mitigate any deleterious effect of the Breach; and
(vi) Provide such other information, including a written report, as Company may reasonably request.
Report Contents. Performance Reports must include a response to all objectives included in your subaward. A detailed response is required in the narrative portion for yes/no performance objectives. The narrative must also reflect on accomplishments for the quarter and identify problems with project implementation and address actions being taken to resolve the problems. Additional information may be required if necessary to comply with federal reporting requirements.
Report Contents. As defined in the Service Level Agreement attached as Exhibit H to this Agreement, TRX will prepare (i) a listing of key service activities, and (ii) definitions of measurements of qualitative and quantitative Performance Standards for each such key service activity, and will submit such listings and definitions to WORLDTRAVEL for approval within a reasonable timeframe not to exceed sixty (60) days after the Effective Date of this Agreement. Such service performance levels will be used to measure TRX’s performance of its responsibilities under this Agreement.
Report Contents. ETRX will prepare (i) a listing of key Service activities, and (ii) definitions of measurements of qualitative and quantitative service performance levels for each such key Service activity including as detailed in the SLA (“Service Performance Levels”), and will submit such listings and definitions to the JOC for approval. The Service Performance Levels will be used to measure HR’s and ETRX’s performance of their responsibilities under this Agreement.
Report Contents. Reports referenced in Paragraph III.11, above, shall 2 include, at a minimum, a detailed description of the following:
(a) Compliance efforts which have been made since the last report with 4 respect to each of the substantive terms of this Agreement, and the 5 actions taken to ensure the City’s own compliance and to require and 6 ensure its Subrecipients’ and Owners’ compliance with Section 504, the 7 ADA, Section 11135, and the terms of this Agreement.
Report Contents. The Qualified Public Depository shall submit to the Treasurer, on a form prescribed by the Treasurer, a monthly report. The report shall include, but shall not be limited to, the following: average daily balance of all demand deposits; the average daily balance of all time deposits and the average daily balance of all savings deposits held by it during the month; the average monthly balance of all public deposits held for the previous twelve (12) calendar months; and such other information requested on the form.
Report Contents. The Qualified Public Depository shall file with the Treasurer, on a form prescribed by the Treasurer, a report of any change of name, address, charter or ownership of the Qualified Public Depository.

WTT will prepare (i) a listing of key service activities, and (ii) definitions of measurements of qualitative and quantitative service performance levels for each such key service activity, and will submit such listings and definitions to the JOC for approval. Such service performance levels will be used to measure WTP's and WTT's performance of their responsibilities under this Agreement.
Report Contents the report provides an overview of the current state of practices for project-level analysis. The findings in this section are based on a review and discussion with sixteen state DOTs that provide procedures for conducting project-level analyses, beyond federal guidance.
