Routine Requests for PTO. Requests for PTO will be resolved based upon seniority or scheduling within a specific department/neighborhood. PTO requests must be submitted and approved by the employee’s departmental supervisor, and by staffing (if required) before becoming effective. All requests must be submitted in writing by the tenth (10th) of the preceding month, in order to be reflected on the monthly schedule posted by the twentieth (20th) of the month preceding the month in which the schedule is effective. Subsequent requests for PTO, during the current or succeeding month, should be made as far in advance as possible. The Employer will notify the employee within ten (10) business days, as to whether the request is granted. For denied requests, the Employer shall provide a written explanation. Reasonable effort will be made by the Employer to approve requests for PTO.

In addition, you understand and agree that we may, from time to time, request and review credit reports and other information about you prepared by consumer reporting agencies or others.

  • Requests for Leave Except as provided under Section B.3., Family Care/Medical Leave Notification, requests for leaves of absence and extensions, with or without pay, shall be submitted in writing to the University. Such requests shall be submitted sufficiently in advance of the requested leave date to provide the University time to assess the operational impact of granting the request. All requests for leaves of absence shall contain the requested beginning and end date of the leave, and any additional information as required.

  • Voluntary Requests for Assistance The Employer shall take no adverse action against an employee who voluntarily seeks treatment or counseling. The Employer shall assist an employee seeking assistance by making available means by which referrals or treatment may be obtained (Employee Assistance Program). Such assistance shall be obtained at the employee's expense.

  • Service Requests In support of services outlined in this Agreement, the Service Provider will respond to service related incidents and/or requests submitted by the Customer within the following time frames:  0-8 hours (during business hours) for issues classified as High priority.  Within 48 hours for issues classified as Medium priority.  Within 5 working days for issues classified as Low priority. Remote assistance will be provided in-line with the above timescales dependent on the priority of the support request.
