Requests for PTO Sample Clauses

Requests for PTO. PTO may be used as allotted and requires prior approval from the employee’s immediate supervisor. The immediate supervisor has the discretion to approve or deny a request for PTO based on operational needs, as the needs of the University come first when scheduling PTO for an employee. To the extent possible, however, consideration will be given to any specific requests by an employee; competitive requests for the same time off shall be decided in favor of the employee with the greater University seniority.

Requests for PTO. Requests for PTO will be governed by the provisions described below.
Requests for PTO. 1. For requests and use of PTO not secured through the bidding process, employees shall make their request on a PTO request form as far in advance as possible, but at least fourteen (14) days in advance of the first day of leave requested. Leave requests with less notice will be considered, but may be less likely to be granted due to short notice. 2. Management will approve leave requests as received, on a first come first serve basis. 3. PTO, or LWOP if paid leave is exhausted, may be used when notice is given to management twenty-four (24) hours in advance of the need to be absent from work for short-notice medical appointments.
Requests for PTO covering in any part the months of June, July and August shall be submitted no earlier than January 1 of the same calendar year. All such requests submitted prior to March 1 will be granted based on seniority, except that nurses shall not be eligible to exercise such right of seniority over less senior nurses if they exercised that right for the same period of time off (i.e. for a period within the same calendar month) during the preceding year. Requests for June, July and August PTO received after March 1, will be based on earliest date of request.
Requests for PTO. PTO, as with all other time off, must, except in unusual circumstances, be requested in writing in advance of the time off desired, and approved or denied in writing within ten (10) calendar days of receipt of the request by the Supervisor. Approval will be based upon the Hospital's determination of its staffing needs, with first consideration given to the needs of particular nursing units. a. When time off is requested without prior approval due to an emergency or illness, a specific reason for the request is to be given and accrued PTO must be used. Such mandatory utilization of PTO shall be limited to a maximum of eight (8) days per calendar year. b. The employee requiring time off without prior approval must call in two (2) hours before the start of the assigned shift. If the employee does not have approval for each day of absence, it shall be considered an unpaid, unexcused absence. Such absences can become cause for disciplinary action. The Hospital may request a doctor's certificate of illness and/or require the employee to be seen by the Hospital's Employee Health Coordinator if the amount of time off due to illness is deemed excessive by the Hospital.
