Short Term Accommodations. A disabled nurse who has obtained medical clearance from her treating practitioner to RTW will provide the ACMS with this documentation (Attending Practitioners Report or Functional Ability Form) including any restrictions and/or limitations. The nurse will advise her manager that she wishes to RTW. The ACMS will consult with the nurse, manager, and Union to develop and communicate the RTW plan.
