Staffing Request and Documentation Form (SRDF). The SRDF is intended as a tool for nurses to document when a unit is not in compliance with its staffing plan. There can be numerous reasons for non-compliance, including but not limited to, short staffing, high acuity, inadequate training, missed meals or breaks, or the wrong patient population. The electronic form is available at every computer provided by the Employer. The form will be transmitted to management, the Staffing Committee, and a representative of the Association. The appropriate manager of the unit will resondrespond to the concern within fourteen (14) days. Prior to completing an SRDF, nurses are encouraged to notify their manager, house supervisor/house coordinator, or charge nurse of staffing issues as close as possible in time to the event(s) giving rise to the staffing concern, and if practical prior to submitting an SRDF.
