Time off Request. ‌ All requests for time off must be submitted through mutually agreed upon shift scheduling software/program. This shall be the master schedule of time off. Time off Request. If an employee, within a six (6) month period, requests time off whether it is holiday time, compensatory time, or vacation, the Department has an obligation to make a good faith response to the employee’s time off request within one (1) week of when the request is made. The employer will not automatically deny these requests in an effort to avoid responding to them in a timely fashion.
Time off Request. The employee is responsible for knowing the balance of their various time-off accounts, which may be obtained via their payroll check stub. If an employee is unsure as to the balance of their various time-off accounts, they should contact the Department Director directly. If the Department Director is unavailable, the employee shall leave a slip asking for their balance. All time-off requests must be submitted in writing, using the Benefit Time Request section of the Village’s Benefit Time Request /Absence from Work Report Form and submitted to the employee’s supervisor. For all time off from work, upon return to work, the employee must fill out the Absence from Work Report section of the Village’s Benefit Time Request/Absence from Work Report Form, and give it to the employee’s supervisor.
Time off Request. 65.01 Time off requests must be made through the Employee Self Service Kiosk (ESS) twenty-four (24) hours prior to the applicable schedule being posted. The time off request will be approved or denied and displayed on the Employee Self Service Kiosk (ESS) within five (5) days of the original submission of the request. With the approval of the Manager, Supervisor or departmental designate, the time period may be abridged to forty-eight (48) hours provided the days off requested are available.

  • Waiver Request Either Party may, in accordance with the Notice provision set out in Article A17.0, ask the other Party to waive an obligation under the Agreement.

  • Leave Request 1. The employee will provide notice to the University that includes the anticipated dates of absence and the type of leave the employee wishes to use during the absence. When the request for leave is for medical reasons, the employee will also provide a doctor’s statement to the University.

  • Requesting Price Increase/Required Documentation Contractor must submit a written notification at least thirty (30) calendar days prior to the requested effective date of the change, setting the amount of the increase, along with an itemized list of any increased prices, showing the Contractor’s current price, revised price, the actual dollar difference and the percentage of the price increase by line item. Price change requests must include H-GAC Forms D Offered Item Pricing and E Options Pricing, or the documentation used to submit pricing in the original Response and be supported with substantive documentation (e.g. manufacturer's price increase notices, copies of invoices from suppliers, etc.) clearly showing that Contractor's actual costs have increased per the applicable line item bid. The Producer Price Index (PPI) may be used as partial justification, subject to approval by H-GAC, but no price increase based solely on an increase in the PPI will be allowed. This documentation should be submitted in Excel format to facilitate analysis and updating of the website. The letter and documentation must be sent to the Bids and Specifications manager, Xxxxxxx Xxxxxx, at Xxxxxxx.Xxxxxx@x-xxx.xxx Review/Approval of Requests If H-GAC approves the price increase, Contractor will be notified in writing; no price increase will be effective until Contractor receives this notice. If H-GAC does not approve Contractor’s price increase, Contractor may terminate its performance upon sixty (60) days advance written notice to H-GAC, however Contractor must fulfill any outstanding Purchase Orders. Termination of performance is Contractor’s only remedy if H-GAC does not approve the price increase. H-GAC reserves the right to accept or reject any price change request.

  • Time Off During Notice Period During the period of notice of termination given by the employer, an employee shall be allowed up to one day's time off without loss of pay for the purpose of seeking other employment.   • Vacation Requests Regular employees become eligible for paid vacation leave once they have completed six (6) months of continuous employment. Vacations requests must be submitted by no later than January 31st to be scheduled for the vacation calendar year of April 1- March 31. Vacation time may be divided into blocks of one to two weeks in duration. Scheduling of vacation shall be in accordance with seniority within a classification. Where an employee chooses to split their vacation, they shall exercise seniority rights in the choice of the first vacation period. Seniority shall prevail in the choice of the second vacation period but only after all other first vacation periods have been selected. Remaining vacation periods will be scheduled in a fair and equitable manner amongst employees within a classification. The approved vacation schedule will be posted at the worksite on or before February 28th in each year. Employees will be limited to booking a maximum of two (2) consecutive weeks of vacation during prime vacation period (June 15 - September 15; and December 15 - January 5), unless further consecutive weeks would not interfere with the vacation preferences of less senior employees. The Employer will make every effort to accommodate requests for vacation of more than two (2) consecutive weeks outside of prime vacation periods. Employees failing to exercise their right to request vacation within the vacation selection time posted by the Employer will forfeit their seniority rights with respect to choice of vacation time. In such cases, the Employer reserves the right to schedule vacation time for the employee. A maximum of two (2) weeks of vacation time may be carried forward from one year to another. Employees who wish to carry vacation forward should notify the Employer by February 28th. If the employee has not requested all of their vacation leave, the Employer reserves the right to schedule the remaining vacation days within the last four (4) months of the calendar year. The Employer may also pay out unused vacation credits at the end of March each calendar year.

  • Bona Fide Request/New Business Request Process for Further Unbundling 6.1 BellSouth shall, upon request of <<customer_name>>, provide to <<customer_name>> access to its network elements at any technically feasible point for the provision of <<customer_name>>'s telecommunications service where such access is necessary and failure to provide access would impair the ability of <<customer_name>> to provide services that it seeks to offer. Any request by <<customer_name>> for access to a network element, interconnection option, or for the provisioning of any service or product that is not already available shall be treated as a Bona Fide Request/New Business Request (BFR/NBR), and shall be submitted to BellSouth pursuant to the BFR/NBR process.
