REQUESTS FOR TIME OFF a. Requests for Union leave with the Company will be made to the Director of Maintenance or his designee. Each such request shall be submitted via the Company email system, facsimile, hand delivery or by US Mail no later than two (2) weeks before the first requested day off. b. The Union shall notify the Company promptly when leaves are cancelled or terminated before the effective date of the Union leave.

REQUESTS FOR TIME OFF. Requests for time off to pursue trade union duties or activities should be made through the representative’s supervisor, department manager, or other appropriate person. Requests for time off should include the purpose of such time off, the intended location and the timing and expected duration of the absence. When considering a request for time off, management should give due regard to the reasonableness of the request, bearing in mind the need for adequate safety cover and the maintenance of the Employer Signatory’s services. The management concerned has the right to refuse a request for time off, provided that the reasons for refusal are reasonable and are made clear to the individual. Time off for trade union duties will not normally be refused unless there is a significant operational issue.
REQUESTS FOR TIME OFF. (A) Normally individual vacation days and personal leave days will be requested five (5) or more days in advance, but an Employee may request such time with twenty-four (24) hours notice for each day requested, except less for an emergency situation. Such vacation days and personal leave days will be granted whenever patient/client care needs permit, provided, however, that while the number of Employees who will be absent on a specific day may certainly be a factor in granting or denying a request, the possibility that some voluntary overtime coverage may be required shall not be the basis for denying such request. (B) Except for the provisions of Section Six, management shall either approve or deny an Employee's request for vacation and personal leave days within seventy-two (72) hours of receipt of the said request. When such request is received with less than seventy-two (72) hours notice, management shall either approve or deny the said request as soon as possible. (C) Management and the Union at each individual facility are encouraged to meet and devise specific procedures and guidelines for implementation of the process set forth in subparagraphs (A) and (B) of this Section. (D) If management and the Union are unable to resolve a dispute arising out of a specific request for either vacation days or personal leave days, the matter will be expedited to Step 2 of the Grievance Procedure. (E) All paid leave time, including sick leave, shall be charged against the Employee's leave records on an hour for hour basis. (F) An Employee may take earned holidays, vacation or personal leave days in conjunction with one another or in conjunction with the Employee's pass days, unless it interferes with patient/client care.
REQUESTS FOR TIME OFF. Employee requests for time off must be made in writing to the employee’s manager at least fourteen (14) calendar days but no more than twelve (12) months in advance prior to the posting of the affected work schedule, unless there are unforeseen circumstances. Employees must note on the request whether they will have enough Earned Leave for the duration of the requested time off. All requests will be posted in the department and held open for a minimum of five (5) calendar days. The Manager shall respond in writing within ten (10) calendar days after the posting period with either a denial of the request, or a tentative approval subject to serious unforeseen staffing deficiencies. Although the Hospital attempts to grant such requests for planned days off, scheduling of such requests is determined by the needs of the Hospital as determined by the Manager. Such requests shall not be unreasonably denied. Exceptions to the above deadlines will be considered when it is critical that travel plans be made well in advance. Proof of reservations may be required (e.g., plane tickets, documentation of reservation deadline, etc.). In the event two (2) or more employees request the same period of time off, the most senior employee shall have a one-time preference. An employee cannot exercise this right of seniority more than once in a three (3) year period. Once an employee’s request for time off has been granted, that request will not later be rescinded due to the fact that a more senior employee later requests the same time off. Employees who are unable to report to work as scheduled shall advise their Manager at the earliest possible time, but no later than one (1) hour prior to the start of the shift, of their inability to report to work. For extended illnesses (including on-the-job injuries), for which medical documentation indicates the absence will exceed fourteen (14) calendar days, employees must contact their Managers weekly as described above.
REQUESTS FOR TIME OFF. 222. A. Revenue Operations will grant requests for paid and unpaid time off (single days) as long as they are requested within five (5) working days of the requested time off. Permission will be limited on any one day to the first requesting employee in the Processing Unit; the first requesting employee in the Day Unit; and the first two (2) requesting employees in PM Unit.

Requests for vacation will be processed consistent with Article VIII (Vacation). 5.1.2 Department Heads will provide notice of unavailability for scheduling by requesting leave without pay through the Employer’s timekeeping system. 5.1.3 At the time the request is made, the Department Head shall recommend an available Relief Department Head who can perform the duties required to the satisfaction of the Employer and third parties paying for services. In the event of a schedule change with less than two weeks notice, the Department Head is still responsible for recommending an available Relief Department Head.

For the May through August Board Period a minimum of eight (8) employees shall be granted time off on any day in this period. Two (2) slots will be reserved for single day vacation sign-ups to be signed prior to the start of the board period, based on seniority. The remaining four (4) slots in the September to April period or six (6) slots in the May through August period, will be signed on a first come first served basis, after the start of the board period. Any additional request shall be determined based on workforce requirements.
