USE OF THE FORM. This form is designed to assist both in formulating and in recording an accurate appraisal of the individuals efforts toward fulfilling requirements which relate to teacher and support personnel responsibility and performance. This form also allows for the recognition of excellence without limitation. Three copies of this form are completed: one for the individual teacher or support personnel; one for the evaluator; and the third to be reviewed by the Superintendent and placed in the personnel file. The personnel file has restricted access and documents are not released without the individuals permission. Anyone who disagrees with any or all of the evaluation may attach a written statement. EVALUATION OF SCALE The "Needs Improvement" and "Unacceptable" range indicate the need for specific improvements and requires comment to identify specific changes needed. USE OF THE FORM. The “As-Is” Residential contract for Sale and Purchase (“As-Is Form”) has become the more widely used version of the Contract. The primary difference between the As-Is Form and the Standard Form is the absence of any obligation on the part of the seller to conduct General Property Repairs, WDO Repairs and permit remediation as required by Paragraph 12 of the Standard Form. As previously discussed, the Standard Form can be converted to an As-Is Form by utilizing Comprehensive Rider K (As-Is). In addition, the Standard Form can be modified through the use of Comprehensive Rider L (Right to Inspect and Right to Cancel). Rider L combines the inspection/cancellation rights with the repair and remediation obligations contained in Paragraph 12 of the Standard Form. Whether the Standard Form or the As-Is Form is used is typically a matter of negotiation between the buyer and the seller.

  • Use of the Card 1. The Cardmember must sign the Card in ink, using a ball point pen, as soon as he or she receives it; the Cardmember must also safeguard the Card and preserve any PIN in extreme secrecy and keep it separate from his or her Card. The Cardmember must not use the Card after the expiration of the validity period embossed on it, and not use the Card after it has been damaged, withdrawn or cancelled.
