Common use of Voluntary Group Life Insurance Clause in Contracts

Voluntary Group Life Insurance. Voluntary Group Life Insurance will be made available in units of $10,000 to a maximum of $150,000 per employee. Spousal coverage will also be made available provided that the amount of coverage purchased does not exceed the amount purchased for the employee. Evidence of insurability satisfactory to the carrier must be provided for: (a) new employees who apply for coverage in excess of $30,000; (b) any existing employee who applies for additional voluntary group life insurance; (c) all applications for spousal coverage. The full cost of this Voluntary Group Life Insurance will be borne by the employee with the Employer making the necessary payroll deductions.
