WORK ORDERS AND REPORTING REQUIREMENTS. (a) Upon the GLO’s identification of specific work within the scope of this Contract, the GLO will select the most highly qualified provider of those services on the basis of demonstrated competence and qualifications. If the GLO selects Provider to perform the work, the GLO may request Provider submit a proposal to complete the work. Provider’s proposal must include pricing and a project plan listing tasks and Deliverables, as appropriate, with a completion schedule or due date for each task and Deliverable. The GLO will negotiate with Provider a Work Order at a fair and reasonable price. Provider must fulfill all applicable requirements in the Contract and applicable Work Order, including providing proof of insurance coverage before beginning the work. (b) If the GLO selects Provider to perform work under this Contract and successfully negotiates with Provider a fair and reasonable price for such work, the GLO will issue a Work Order to Provider. The GLO may issue multiple Work Orders during the term of this Contract. Each Work Order must be in writing and signed by the Partiesauthorized representatives. Each Work Order must include, as applicable: a scope of services; a list of tasks; a completion schedule; a list of Deliverables and a due date or completion schedule for each Deliverable; a detailed budget; and such other information or conditions necessary for the work described therein. Upon execution by both Parties, each Work Order shall become an Attachment hereto, with this Contract and all Attachments thereafter being referred to collectively as the “Contract.” (c) The GLO does not represent or guarantee that it will issue Work Orders to Provider pursuant to this Contract. All work requested under this Contract will be required on an irregular and as-needed basis throughout the Contract term. The GLO makes no guarantee of volume or usage under this Contract. (d) Provider must submit reports as required by any applicable Work Order.

All such Work Orders shall be in writing and signed by the Partiesauthorized representatives. (b) The GLO shall attempt to negotiate with Provider a fair and reasonable price for completing a Work Order. Upon successful negotiation of a fair and reasonable price, the GLO shall issue the Work Order to Provider. (c) Each Work Order shall include a scope of services; a list of tasks; a task completion schedule; a list of Deliverables, if any, and a due date for each Deliverable; a detailed budget; and other information or conditions necessary for the work described therein. Upon execution by both Parties, each Work Order shall become an Attachment hereto, with this Contract and all Attachments thereafter being referred to collectively as the “Contract.” Provider may not begin work or incur costs related to a Work Order before supplying the GLO with documentation that, in the GLO’s sole judgment, demonstrates that Provider obtained all insurance specified in the Work Order. (d) The GLO makes no guarantee of Work Order volume or usage under this Contract. The GLO does not guarantee that it will issue Work Orders to Provider for any of the tasks set forth in Section 1.03. The GLO will request work under this Contract on an irregular and as-needed basis throughout the Contract term. (e) Provider must submit requested reports as directed by the applicable Work Order.

  • Subsidy Requests and Reporting Requirements 1. The Grantee or Management Company shall complete a CRF Subsidy Request Report - Recap of Tenant Income Certification, which provides a unit-by-unit listing of all units in the Development for whom assistance is being requested and gives detailed information including the occupants’ eligibility, set-aside requirements, amount of household rent paid, utility allowance and amount of CRF Rental Subsidy requested. 2. The CRF Subsidy Request Report - Recap of Tenant Income Certification shall be prepared as of the last day of each calendar month during the period of performance and shall be submitted to XXXXxxxxxxxx@XxxxxxxXxxxxxx.xxx and Florida Housing’s monitoring agent no later than the 15th day of the following month. The December 2020 request will be due on or before December 15th. The Grantee will submit executed Coronavirus Relief Fund Rental Assistance Applications and supporting documentation to Florida Housing’s monitoring agent within 5 days upon the monitoring agent’s request.

  • General Reporting Requirements The MA-PD Sponsor agrees to submit to information to CMS according to 42 CFR §§423.505(f), 423.514, and the “Final Medicare Part D Reporting Requirements,” a document issued by CMS and subject to modification each program year.

  • Child Abuse Reporting Requirements A. Grantees shall comply with child abuse and neglect reporting requirements in Texas Family Code Chapter 261. This section is in addition to and does not supersede any other legal obligation of the Grantee to report child abuse. B. Grantee shall use the Texas Abuse Hotline Website located at xxxxx://xxx.xxxxxxxxxxxxxx.xxx/Login/Default.aspx as required by the System Agency. Grantee shall retain reporting documentation on site and make it available for inspection by the System Agency.

  • EDD Independent Subrecipient Reporting Requirements Effective January 1, 2001, the County of Orange is required to file in accordance with subdivision (a) of Section 6041A of the Internal Revenue Code for services received from a “service provider” to whom the County pays $600 or more or with whom the County enters into a contract for $600 or more within a single calendar year. The purpose of this reporting requirement is to increase child support collection by helping to locate parents who are delinquent in their child support obligations. The term “service provider” is defined in California Unemployment Insurance Code Section 1088.8, Subparagraph B.2 as “an individual who is not an employee of the service recipient for California purposes and who received compensation or executes a contract for services performed for that service recipient within or without the State.” The term is further defined by the California Employment Development Department to refer specifically to independent Subrecipients. An independent Subrecipient is defined as “an individual who is not an employee of the ... government entity for California purposes and who receives compensation or executes a contract for services performed for that ... government entity either in or outside of California.” The reporting requirement does not apply to corporations, general partnerships, limited liability partnerships, and limited liability companies.
