In addition to the records required to be maintained by Title 22 CCR, CONTRACTOR shall also maintain the following information in Xxxxxx Youth’s/NMD’s case files: Information regarding the Xxxxxx Youth’s/NMD’s participation in TPSP/ILP, as applicable; Statement of behaviors with potential risk and/or safety concerns; IPC referral for STRTP placement and/or admission statement; Initial and monthly CFT reports; Xxxxxx Youth’s/NMD’s financial information, including disbursements for clothing and property signed for by Xxxxxx Youth/NMD, and allowances received by, and signed for by, Xxxxxx Youth/NMD; CJC0821-00 Page 20 of 34 February 9, 2021 Diagnostic studies; Intake interviews conducted with Xxxxxx Youth/NMD; Special Incident Report; Written quarterly evaluations; Child and Adolescent Needs and Strengths assessment and/or other mental health assessments; Mental health treatment plan and mental health program progress notes; Records of medical/dental visits and treatment, including a copy of CHDP physical or its equivalent that is less than one (1) year old; Xxxxxx Youth/NMD’s monthly weight chart; Psychotropic medication orders/scripts and forms regarding psychotropic medication, including, but not limited to, JV-220, JV-220(A), JV-220(B), and court approved JV-223; Monthly feedback from the Xxxxxx Youth’s/NMD’s school regarding academic and social performance; and Xxxxxx Youth’s most current IEP.

Related to Xxxxxx Youth/NMD Records

  • Client Records 26.2.1 CONTRACTOR shall prepare and maintain accurate and complete records of clients served and dates and type of services provided under the terms of this Contract in a form acceptable to ADMINISTRATOR.

  • Transit Records 7.6.1 CenturyLink and CLEC will exchange wireline network usage data originated by a wireline Local Exchange Carrier (LEC) where the NXX resides in a wireline LEC Switch, transits CenturyLink's network, and terminates to CLEC's network when Technically Feasible and commercially reasonable. Each Party agrees to provide to the other this wireline network usage data when CenturyLink or CLEC acts as a transit provider currently or in the future. The Parties understand that this information is Carrier protected information under Section 222 of the Telecommunications Act and shall be used solely for the purposes of Billing the wireline LEC. CLEC will provide to CenturyLink information to enable CenturyLink to provide transit records on a mechanized basis when Technically Feasible. This includes, but is not limited to: service center information, operating company number, and state jurisdiction. CenturyLink and CLEC agree to exchange wireline network usage data as Category 11-01-XX.

  • Patient Records Upon termination of this Agreement, the New PC shall retain all patient dental records maintained by the New PC or the MSO in the name of the New PC. During the term of this Agreement, and thereafter, the New PC or its designee shall have reasonable access during normal business hours to the New PC's and the MSO's records, including, but not limited to, records of collections, expenses and disbursements as kept by the MSO in performing the MSO's obligations under this Agreement, and the New PC may copy any or all such records.

  • Health Records Provider agrees to cooperate with Subcontractor and/or Health Plan to maintain and share a health record of all services provided to a Covered Person, as appropriate and in accordance with applicable laws, regulations and professional standards.

  • Return of University Records Contractor agrees that within thirty (30) days after the expiration or termination of this Agreement, for any reason, all University Records created or received from or on behalf of University will be (1) returned to University, with no copies retained by Contractor; or (2) if return is not feasible, destroyed. Twenty (20) days before destruction of any University Records, Contractor will provide University with written notice of Contractor’s intent to destroy University Records. Within five (5) days after destruction, Contractor will confirm to University in writing the destruction of University Records.

  • Personal Data, Confidentiality, Recording of Telephone Calls and Records 22.1. The Company may collect client information directly from the Client (in his completed Account Opening Application Form or otherwise) or from other persons including, for example, the credit reference agencies, fraud prevention agencies, banks, other financial institutions, third authentication service providers and the providers of public registers.

  • Medical Records Medical records relating to Trial Subjects that are not submitted to Sponsor may include some of the same information as is included in Trial Data; however, Sponsor makes no claim of ownership to those documents or the information they contain. c.

  • Personnel Records (A) There shall be only one official personnel file for each employee, which shall be maintained by the employing agency. Information in an employee’s official personnel file may be maintained in electronic as well as paper form.
