Health Records. Provider agrees to cooperate with Subcontractor and/or Health Plan to maintain and share a health record of all services provided to a Covered Person, as appropriate and in accordance with applicable laws, regulations and professional standards.

Health Records. Contractors must maintain current and confidential (A-13) health files on all enrolled children that include: Medical and dental history. Immunization records. Allergy information. Food preferences and restrictions. Health screening results (D-7). Dental screening results (D-8). Medical examination records (D-9). Accident reports. Documentation of health-related family contacts. Contractors must maintain a tracking system to ensure these records are kept up-to-date. Contractors must maintain documentation that they: Notify parents that staff are mandated reporters of suspected child abuse and neglect in accordance with RCW 26.44 (D-3). Notify parents, employees, and any other interested parties 48 hours in advance of the application of pesticides in accordance with RCW 17.21. Notification is not required if children will be out of the facility for two consecutive days after application. Notify parents that children have been exposed to infectious disease and parasites in accordance with WAC 170-295-3010.
Health Records. Every school must comply with all department of public health requirements. Every school shall have on file a health record for each student enrolled in the school. Upon a student's transfer, student health records are forwarded with the student transcript to the receiving school. Schools shall comply with all Family Educational Rights and Privacy Act rules as applicable. For additional information on student records, see Transfer of Records.
Health Records. Contractor shall maintain adequate mental and/or medical health records of each individual client/patient which shall include a record of services provided by the various professional personnel in sufficient detail to make possible an evaluation of services, and which shall contain all necessary data as required by NCBH and State or federal regulations, including but not limited to records of client/patient interviews and progress notes.
Health Records. You acknowledge and agree that the Software is not an electronic health or medical record. You further acknowledge and agree that the Software should not be used as a primary means of documenting health information or relied upon for storing official health or medical record documentation. You acknowledge and agree that you are solely responsible for maintaining official health or medical records documentation in accordance with all applicable laws and will not rely upon the Patient Data transmitted to, and stored by, the Recipients for meeting your obligations under such laws, if any.
Health Records. The Provider must create, maintain, store and retain Service User health records for all Service Users. The Provider must retain Service User health records for the periods of time required by Law and securely destroy them thereafter in accordance with any applicable Guidance. The Provider must: use Service User health records solely for the execution of the Provider’s obligations under this Contract; and give each Service User full and accurate information regarding his/her treatment and Services received. The Provider must at all times during the term of this Contract have a Caldicott Guardian and shall notify the Authority of their identity and contact details prior to the Service Commencement Date. If the Provider replaces its Caldicott Guardian at any time during the term of this Contract, it shall promptly notify the Authority of the identity and contact details of such replacements. Subject to Guidance and where appropriate, the Service User health records should include the Service User’s verified NHS number.
Health Records. All children entering either kindergarten or first grade, and those entering sixth grade, must have on file by the first student attendance day in August of the current year a completed written health exam by a physician. Dental exams are required for grades K, 2 and 6, and must be turned into the office by May 15 of the school year.
Health Records. Medical examination forms and immunization forms are required for all kindergarten students and all new students (grades 1-8). Medical records are also required for participation in any school sponsored sports program. Record of all immunizations required by law must be on file in the school office. A student will not be permitted to start classes until all required health forms have been completed by a physician and returned to the school office.
Health Records. Provider shall maintain all Member health records in the manner required under applicable state and federal law. Additionally, Provider shall maintain adequate medical, financial, and administrative records related to Covered Services rendered by Provider under this Agreement. In order to monitor and evaluate the quality of care, conduct evaluations and audits, and to determine on a concurrent or retrospective basis the necessity of and appropriateness of Covered Services provided to Members, UBH shall have access to such information and records. Provider shall also make these records available to appropriate state and federal authorities involved in assessing the quality of care or in investigating Member grievances or complaints.
