HealthCare Plan definition

HealthCare Plan means the document contains written instructions about a specific health condition including the when and how specific interventions are to be carried out in a school or child care setting. This document should be signed by the child’s health care provider and parent. Health Care Plans can be collaboratively created by the child care health consultant, the child’s parents, health care provider and center staff, and are necessary for the care of children with chronic health care conditions such as asthma, seizure disorder, diabetes, or severe allergy. Health Care Plans may also guide the care of children with acute conditions that may need short-term special management in the group care setting such as child returning to care with a cast, or after a surgical intervention.
HealthCare Plan means any hospital or medical insurance policy or certificate, healthcare plan contract or certificate, qualified higher deductible health plan, health maintenance organization or other managed care subscriber contract, or state healthcare plan. This term shall not include limited benefit insurance policies or plans listed under paragraph (3) of Code Section 33-1-2, air ambulance insurance, or policies issued in accordance with Chapter 21A or 31 of this title or Chapter 9 of Title 34, relating to workers’ compensation, Part A, B, C, or D of Title XVIII of the Social Security Act (Medicare), or any plan or program not described in this paragraph over which the Commissioner does not have regulatory authority. Notwithstanding paragraph (3) of Code Section 33-1-2 and any other provision of this title, this chapter this term shall include stand-alone dental insurance and stand-alone vision insurance for purposes of this chapter.

HealthCare Plan means any plan whereby any person or entity undertakes to provide, arrange for, pay for or reimburse any part of the cost of any health care services.
HealthCare Plan means one or more applicable healthcare benefit plan as sponsored and maintained by the Employer from time to time for its nonrepresented active or retired salaried employees, (including both pre- and post-Medicare coverages).
HealthCare Plan or "health benefits" mean the healthcare plans for medical, prescription drug benefits and dental. If the Employer is required by law to provide vision or other healthcare benefits not otherwise heretofore provided, the definition of "healthcare plan" or "health benefits" shall also include the additional mandated benefits. Employee contributions shall be made by way of withholding of the contribution from the employee's pay, salary, or other compensation. Withholdings shall be made by way of twenty-four (24) equal payroll deductions in a calendar year, to the extent possible, in accordance with the Employer's customary payroll practices unless otherwise required by law. The amount payable by any employee under this Article shall not under any circumstance be less than 1.5 per cent of base salary. An employee who pays the contribution required in Subsections 2. through 5. above shall not also be required to pay the contribution of 1.5 percent of base salary.
HealthCare Plan or “health benefits” mean the healthcare plans for medical and prescription drug benefits. In the event Federal or State law require the Employer to provide other or additional benefits not heretofore provided, the definition of “healthcare plan” or “health benefits” shall also include the mandated benefits.
