Home Supports definition

Home Supports means services, supports and supervision provided for individuals in
Home Supports means services, supports and supervision provided for individuals in and around their residences up to 24 hours a day, seven days a week (24/7). Services include support for individuals to acquire and retain life skills and improve and maintain opportunities and experiences for individuals to be as independent as possible in their home and community. Services include maintaining health and safety and home modifications required for accessibility related to an individual’s disability, including cost-effective technology that promotes safety and independence in lieu of paid direct support. Home supports shall be in compliance with HCBS rules which emphasize choice, control, privacy, tenancy rights, autonomy, independence and inclusion in the community.

Examples of Home Supports in a sentence

  • Provide services to eligible Advantage waiver members pursuant to O.A.C. 317:30-5-760 through 317:30-5-764, In Home Supports, Community and Homeward Bound waiver members pursuant to 317:30-5-480, 317:30-5-481, 317:30-5-482, 317:35-9-97 and/or for members in the Living Choice Program, O.A.C. 317:30-5-1200, 317:30-5-1201 et seq.

  • In delivering In Home Supports services, providers are required to observe, report and record any change in client’s condition and home environment as part of the overall service delivery.

  • This includes services provided under the HCBS-CFI waiver program, Developmental Disabilities (HCBS-DD) waiver program, Acquired Brain Disorders (HCBS-ABD) waiver program, and In Home Supports (HCBS-I) waiver program.

  • Any requests for planned PTO after the fourteen (14) day period will be considered by the Executive Director, or Program Director of Home Supports or Home/Community Support Manager.

  • The Executive Director, Program Director of Home Supports or Home/Community Supports Manager reserves the right to deny requested planned PTO periods due to the operational needs of that facility.

Related to Home Supports

  • Helpdesk Support means the 24x7x365 centre which shall handle Fault reporting, Trouble Ticketing and related enquiries during this contract.

  • Level 1 Support means qualifying and logging all Technical Support Incidents, answering technical inquiries via telephone support and email regarding the Work and performing limited diagnostic services.

  • SAP Support means SAP's then current support offering made available to Customer, as stated in the Order Form.

  • Support means interventions provided as a supplement to those regularly provided by the LEA, including services provided in accordance with an IEP, that are designed to meet students’ needs for behavioral, social, emotional, and other integrated student supports, in order to enable students to engage in, and benefit from, the supplemental instruction being provided.

  • Customer Support means the services by which 21Vianet may provide assistance to Customer to resolve issues with the Services.
